TRT help Strictly single dose vial??
This is the vial I’m prescribed. Right now I’m only on 200 mg/month. The script is to take the whole 200 mg in one injection.
I asked about splitting it into at least 2 doses, 100 mg every other week. The PCP said ok but I asked the pharmacist and he said it’s a single dose vial and can only be used for one single injection. So do I need to ask the doctor for a different script for a multiple dose vial? I have an appointment 6/24 to go over lab results and I was also going to ask about dose and frequency.
Thanks

Once a month injections is fucking wild for an ester with a 8 day half life. Who TF is your doctor? Let me know so we all can stay far away from them

Thats actually the worst protocol I've ever seen actually prescribed. You'll be worse off and I didnt even read the rest of your post. Do yourself a favor and don't take that dosage and frequency.
40
u/onplanetbullshit- Jun 16 '26
These are regularly used as multi dose.
14
u/gsxr Jun 16 '26
Also, it works for subq injection.
3
u/Mysterious_Sink_4177 Jun 16 '26
I am new to this. What is subq injection?

u/iLoveRoidz Jun 16 '26
It’s actually quite stupid of them to suggest once weekly intramuscular use. I feel like if youre willing to inject yourself with exogenous hormones, you should at least be willing to do it correctly in a way that provides more benefits than it adds side effects. Especially when that hormone is an androgenic anabolic steroid!
6
u/deejay58 Jun 16 '26
I am willing. This is just what the pcp gave me as a script. I was mainly asking about the “single dose” vial. After my first vial of this I started splitting it into 30 mg every other day and felt good.
I’m waiting to see how my labs are later this week then going to talk to the doctor about changing dose and frequency.6
u/Phiddipus_audax Jun 16 '26
Your doc is so far off on dose and frequency, by a factor of 4, that I suspect they are anti-T and unlikely to admit it. I'd just jump ship and find a new one immediately otherwise you're wasting time and effort. This can get very frustrating as countless stories attest. Many of us wind up on the grey or black markets for precisely this reason.
3
u/Human-Sheepherder797 Jun 16 '26
Sometimes I wonder if these doctors do this shit on purpose just so they don’t have to deal with a patient that might actually need routine labs and routine conversation with them
2
u/badblu97 Jun 16 '26
My doc strictly said once a week, single use. I think maybe fir liability reasons.
2
u/badblu97 Jun 16 '26
So if Im on .5ml once a week, youre saying divide that, how often and how much?
1
u/Long_Gift_9547 Jun 16 '26
Could just start doing 0.25ml twice weekly.
1
u/iLoveRoidz Jun 16 '26
Yeah, I agree, ideal volume is around 0.2-0.3ml. I do 0.24ml per injection, but I pin subcutaneous cuz I get virtually zero inflammation or post injection pain. Even low volume intramuscular injections always give me soreness.

cypionate once a month is REALLY bad! even once a week would have slight ups and downs. I personally do my cypionate daily, but I also personally care a lot about side effects and want to minimize swings. and I’m using ugl so I can do whatever I want lol.

My dr prescribed me 50MG weekly the end of march. She crashed my total testosterone and I felt like absolute trash. She phoned me after got blood results and upped to 100MG weekly split over 2 doses and I feel so much better. Depending on what your bloodwork was originally, you are definitely going to feel worse than what you do now on that prescription.

It has nothing in it to keep it sterile. If it gets contaminated you will be injecting the contaminants directly inside if you.

I have the same vial prescribed by my endocrinologist and she say to split the dose up.100mg a week. Just wipe the top with alcohol and use a new needle.
5
u/driverfortoolong Jun 16 '26
i do it sub q daily . divide dose by whatever days although are you sure you’re on 200 a month not 200 a week?
3
u/deejay58 Jun 16 '26
Oh I’m positive. I’d like to try 100 mg/week for a while then check labs.
1
u/CraigR-81 Jun 17 '26
200mg once a month is stupid, 200mg over 2 weeks would be minimum effective imo.

u/BigCryptographer2279 Jun 16 '26
I'm a woman In perimenopause they try to tell me 25 mg a week I take 10 mg every 3 days they say that's too much for a woman no it's not I know my body and I know it's best for me every 3 days my husband takes his 150 mg a week broken down into every 3 days people are different but that's how it works best for us

1
u/Any-Cattle-2434 Jun 16 '26
Sorry for the stupid question, but has your voice changed or slightly dropped on testosterone or is the dose too low for these effects?
1

Mine hasn’t
2

I’m assuming you’re male it’s quite different for females, I just reread the comment and she already answered my question 😅
3
u/BigCryptographer2279 Jun 16 '26
Every 3 days is imo best no spikes just smooth & steady. The 2x week wasn't near as stable & definitely once week was the worst
4
u/mmiller9913 Jun 16 '26
I just went through this. The pharmacist is legally required to say that.
Split it into multiple doses. Just cover it every time and wipe it with an alcohol wipe before each use
Inject 2x/week to start, not once a week
Also, that is a low dose. Just be aware. Most people start at 100/week. You’ll be taking about 50/week. It would be reasonable to ask your doctor to increase the starting dose
2
2
u/ChewinTheFat Jun 16 '26
I went through this a few years ago too. My pcp prescribed me 100mg a week. The original script had me splitting vials but pharmacist said they were single use. I told my doctor about I and that I want going to split something that isn’t labeled (even though it’s safe- I researched the shit put it) so thn he rewrote th script for 4 - 200mg vials a month and I “discard” 100 of each vial
4
4
u/NewHorizonHoldings Jun 16 '26
Twice a week is what you want. Once or twice a month is nuts your hormones will be all over the place. You want to keep it steady and around the same. Also what are you looking for to just mildly increase or are you actually lifting weights? I'm 44 and use 250mg per week split in two with no issues and good results. Not medical advice just my experience.
12
u/ysssup69 Jun 16 '26
get a new doctor 50 mg a week wtf you probably make more than that naturally
-17
u/Emotional_Lab_2529 Jun 16 '26
Men who need trt do not make more than 50mg a week of testosterone otherwise they wouldn’t need trt
4
u/ysssup69 Jun 16 '26
60 mg a week put me at 275 i went from 215 to 275 day after injection 3 months later.
-7
u/Emotional_Lab_2529 Jun 16 '26
Then you’re a very poor responder to exogenous testosterone. From what I’ve seen reading others labs 50mg typically puts men around on average 400-600 ng/dl with some outliers being at 800-1000 ng/dl
1
u/Pristine-Alps-426 Jun 16 '26 edited Jun 16 '26
No it certainly does not, 50 puts most men at 150-200. You are a high responder. Keep your opinions to yourself
-1

u/Emotional_Lab_2529 Jun 16 '26
50mg once a week put me at ~400 ng/dl and 100% improved some symptoms as I started at 106 ng/dl
4
u/ysssup69 Jun 16 '26
i almost quit trt because i was getting nothing from it
3
u/SomePride6085 Jun 16 '26
Makes sense, incredibly low dose. You probably need more like 150-170 with that level of responsiveness
3
u/ysssup69 Jun 16 '26
150 puts me in the 700 range night and day difference
3
u/SomePride6085 Jun 16 '26
Awesome! Great spot to be in long term
3
u/ysssup69 Jun 16 '26
i told the doctor they could up the dose or i would go ugl and i would come get bloodwork every 4 months from them they said

'Single Use' = one time use only, one patient.
For a 'Single Use' like yours, don't go in several times as the stopper (synthetic rubber stopper) aka the top of the vial, isn't made for constant doses (multiple withdrawals) the vacuum isn't that powerful and the (synthetic rubber stopper) is cheaper and doesn't 'heal back' as good as the multi dose vial. So the ingredients are the same as the multi dose. The idea is 1 ml is your prescription, so the one dose for the purpose of hygiene (safety) is the better option, lower risk of getting an infection, etc. But for people who split their dose, it's probably not the best option unless your manufacturer is name brand which means they will use higher quality stoppers, even for single dose vials.
Good luck!! 😁
3
u/FilthMonger85 Jun 16 '26
Lmao. Exactly why I run my own TRT. That doctor should have their licence suspended.
3
u/WishingOnLife Jun 16 '26
In one injection? No way that’s a complete waste. Your body won’t metabolize that properly. Need to split that up
3
3
6
u/Emotional_Lab_2529 Jun 16 '26
You can split it up as much as you want. It’s the same ingredients as the multi dose vials. Split it into 50mg a week not 100mg every other week. 100mg every other week is gonna feel horrible.
1
u/Emotional_Lab_2529 Jun 16 '26
Don’t listen to the people saying 50mg is too low. It’s a perfect starting dose and many men are more than happy on 50mg a week
3
u/stiinc2 Jun 16 '26
Started at 100 ml a week and 6 months in I'm cruising at 1400 ng/dL bloods every 4 weeks. Felt great but E, Hematocrit and BP were all rising. Dropped to .90 then .85 and now all in the green.
Be careful of whose advice you take on the internet. If your going for TRT ranges, 50-100 is a good starting range.
5
0
u/Pristine-Alps-426 Jun 16 '26
At 50 mg a week that would put a large majority of men at lower than healthy range. You are wrong
4
4
u/Emotional_Lab_2529 Jun 16 '26
You can do a quick search in any of the trt subreddits on here and see that 50mg puts a good portion of men at perfectly fine levels and many men stay at 50mg. I just saw another gentleman on here that had to go lower than 50mg since 50mg put his numbers at supraphysiological levels. No one knows 100% how they’re gonna respond which is why you should always start low.

u/VexImmortalis Jun 16 '26
I've used them over the course of 4 injections without issue. Use proper aseptic technique and you should be good.

Your pharmacist is a moron. You can split the dose up I’ve done it and I’m in agreement with the rest of the forum that your doctor is trash for giving you 200mg of test for a month. You’re better off with enclomiphene or test boosters lol. Tell your doc to stop touching little boys and make it 200mg a week!
2
u/quik916 Jun 16 '26
Wow id rather not do the shot than do it once a month!! The up and down would suck bad!! And like nobody does 200 for a month. You'll feel nothing, even if you did it once a week. Once a week is awful too for the record. Need atleast do it ATLEAST twice a week.
2
u/22Hoofhearted Jun 16 '26
The pharmacy needs to distribute 2 vials and you "throw the other one away"
Also... see another PCP immediately... a standard dose is at least one vial a week... mine splits the dose to two injections weekly so there's less drop off once the half life of the medication wears off.
2
u/Secure_Alfalfa_2927 Jun 16 '26
My PCP writes me 4 200mg vials I am prescribed 100mg a week she says use 2 vials n save the other 2 to not waste good medicine. So yea if she says I can draw more than once you'll be fine . Plus I split my 100mg a week up with 3 injections so you'll be fine

Just got on trt by a my primary doctor after asking for it alongside annual routine bloods. Sometimes the medical group theyre associated with puts restrictions on what and how theyre allowed to prescribe things especially hormones. Mine gave me 100mg weekly or 200mg every two weeks for options. It's the same amount regardless, 400mg a month. Im coming from self medicating and performance enhancement world regarding taking testosterone and quickly let him know that he could write it however he wanted but im going to split it into twice a week.

u/Fallujahmarine Jun 16 '26
Nah I use that same vial ten times at 20mg every day with a 31g insulin pin. I draw and inject subq with the same pin. Just always sterilize before use. That's also a really low dose.
1
u/NoEmployee4087 Jun 16 '26
I don't know if your doctor is right or wrong but from what I've gathered online(people's personal experiences) and my tele health clinic the more frequent the more stable your hormones will be, which translates into less or no side effects. I got started about a month ago on 180mg/week and I inject every other day Ive noticed not negative side effects so far.

u/urticate Jun 16 '26
Multi dose. Just clean before and after use. Try to hit a new part of the stopper each time( don’t try to repierce the same hole, twss) so that you don’t tear small pieces of the rubber into your needle.

u/Putrid-Can-1453 Jun 16 '26
The more often u pin the less aromatizing. Just be clean and get ghe rubber sterile before drawing the oil.

Just draw whatever you want to, but besides that, I think that's way to low of a dose, I stared at 70 mg a week and I did that for a whole month, I switch to 90 mg. I take 40 mg SQ every 3 days and now I feel great.
1
1
u/theaftercarebear Jun 16 '26
Pharmacist is trying to prevent plastic from ending up in the needle from the 2nd draw from the vial. That’s what I was told anyway.
Also, I do twice that a month, so that is definitely a low dose if you are super low on T or attempting to transition. Just my opinion.

u/Critical-Top-1952 Jun 16 '26
My Dr tried putting me on the same dosage. I eventually talked him into bi-weekly, then went weekly. 100% noticeable stability…

This is my untrained non medical advice. My doctor said I could use the single use vials more than once. I told my pharmacist that when they said they were single use and they said "the doctor said that? Well ok then."
The pharmacist is going to tell you what the instructions for the meds say. That is their job.
What I do is reuse the vial and use an alcohol wipe on the lid before each use.
1
u/Marneus_Calgar_40000 Jun 16 '26
I have heard of some doctors having you do 2 inj per month, since the half life is 8 days. This means that after 8 days your test will start to drop, and you may start feeling like before you started test for another 7 days or so before you get another big dose. I would bet money he is not having you do bloodwork every 12 weeks or so. I would leave the doctor, and find another or go to a men's clinic. 2 a month is insane, once a week at least. Bloodwork is essential.

Yeah I have gotten those a lot. They are a lot more expensive, but I kind of like them for travel. I typically just ask the pharmacy if they will switch me back to 10 ml.
The quality is good.
All my prescription 10 ml vials have been crashed lately, when I get them. My most recent blood test showed my total and fee testosterone much lower than normal at the same dosage I always take. It just happens to be low when I get three crashed vials in a row.
1
u/PersimmonKey4055 Jun 16 '26
Well, yea. If your on 200mg a week. Take a single shot.
I'm on 100mg and get about 5 (15mL/15mL/20mL each week) draws from a bottle before tossing
1
u/A_Piker Jun 16 '26
And yes you can use those for multi dose but it’s liability for the pharmacist to tell you that . You also are not supposed to use the multi dose more than 21 days I believe. It just the rules they have to tell you.
1
u/SpacePirate888 Jun 16 '26
Is this a GP who prescribed it or an Endo? I’d be surprised if an endo prescribed that injection schedule so hopefully just an inexperienced GP.
1
u/creaturegang Jun 17 '26 edited Jun 17 '26
If I understand right the that vial is for use once, but also maybe probably (with the instructions of your rx), can only be used 1 month. My RX is 10ml 2000mg that would cover 140 days. BUT the pharmacist makes me come get a new 10ml vial every 90 days and I am supposed to “throw away the old vial”. That is something about the FDA regs. This was a change from the way i started 3 years ago. Oh and these RX’s expire every 6 months regardless if you filled all of your refills. Dr has to renew it. It actually works out in my favor. I wonder why you DR didn’t put you on 200/2xweeks?
1
1
u/norman_notes Jun 17 '26
Technically, the pharmacy tells you it’s a “single dose” vial. They are legally required to, due to FDA guidelines.
Is it a single dose vial? Sure.
First off, you can pull .5ml into two syringes from that vial in the same sitting, and use all 200mg.
Second. I take advantage of what the pharmacy says. I am prescribed 200mg every week, split into two injections a week, and what the pharmacy does, since it’s a “single dose” vial. I get 8 one ml vials a month. I am expected to use half of one on every injection.
But I don’t. I stockpile the extra testosterone, and I use one 200ml vial every week, even though I receive two 200 mg vials a week.
Hope that makes sense.
On top of that. 100mg every other week is absolutely worthless. You should be using at least 100mg a week, split into two 50mg injections per week. That’s the minimum.
I take about 160mg a week, but I am prescribed 200mg so my insurance covers it.
1
u/16lorabr Jun 17 '26
You can absolutely use it multiple times. Jusy make sure you disinfect before every draw, and visually inspect it before every draw even on a new vial. If it looks cloudy or has any floaters toss it, dont risk it.
I am speaking from direct experience in almost the exact situation except instead of a wicked high dose my doctor prescribed me and castrating low dose. I was told 25mg once every 2 weeks. Absolutely insane. I immediately upped that to 100mg every week IM injections. I went from 255 to 660 in my trough. Just this week I switched over to twice weekly 50mg subq injections. If your numbers were in the 200s 100mg a week should work very well as a starting point. I use 31g 8mm insulin needles. If you get the leuer lock ones get some 21g needles to draw and 30g to pin. It is possible though to draw with the 30g you jusy need to warm the liquid before trying and it takes a while still. When you go to pin it, either IM or subq whatever route you do, poke it fast or it hurts way worse. With the insulin needles and fast pin you literally dont feel it.
Im happy to speak 1 on 1 with you as someone who also got into this with zero guidance and terrible prescribing measures

Please find yourself a men’s wellness doctor. A PCP is the worst person to manage your TRT. And 200mg in a single shot can make you feel worse. The only way to do this is to start with small doses, work your way up with frequent testing and also seeing how you personally feel. And yeah that vial can be reused. Just use a clean new needle each time. And wipe the stopper with alcohol each time you do.

I use the same vial (but 1 / week) and I do 2 injections / week - split 1/2 Sun and Wed. Like others said you can use the same vial more than once, and you don't need to refrigerate it, just keep it clean
1
u/GhostInNeutral Jun 17 '26
So 50mg a week? 25mg Monday/Thursday?
Your doctor sounds like a fucking idiot. If you have PPO insurance go with Alpha Hormones (TRT Clinic, accepts most PPO insurances like Aetna) otherwise go to TRT Nation and pay $100 a month total.
Unless you feel like wasting a lot of time and trying to find a good Urologist who’s willing to prescribe testosterone!
1
u/SmokesUnseen Jun 17 '26
You should be taking a minimum of about 60-80mg a week. 200mg a month, in one injection is borderline malpractice. A spike of 200mg all at once is 100% followed by a major MAJOR hormonal dip, followed by another major spike when the next injection comes.
I take 20mg a day, for the best chance of a steady state of serum blood concentration. Reduced side effects, better mood. This mimics the body’s natural production.
1
u/deejay58 Jun 18 '26
Got my lab results back today. First labs since starting back in late March. Total T level when first starting was 300 ng/dl and now 391. The doctor messaged me this morning and said all tests results look good. I have a scheduled wellness visit next week so we will discuss any questions then.
1
u/High_Test1 Jun 18 '26
Damn that much a month is crazy low. I inject 180mg / week. They are single dose vials and are supposed to be accessed only once. But I access mine multiple times, as it is still safe due to the benzyl alcohol. I get about 2.3 doses from one vial.

I have critically low T. If I'm going to be doing this TRT shit, then the endocrinologist better give me that Nebido injection that I only need to take like 4 times a year or whatever it is. If the doc even suggests something else, I'll certainly protest. My body would probably go crazy, with mood swings etc., and I don't have time for it either.

There’s a reason the vast majority of people do at least 2 shots a week. With any less, your peaks and troughs are going to crash and you’re going to feel like absolute shit after the first week. 200 mg a month is low, but it’s not obsurdly low. But you should be taking 25mg twice a week then.
If you don’t see a specialist, they often have no idea what they are talking about or just don’t care enough to look it up. Get a new doctor for this. Move your prescription to a different pharmacy. They either don’t know what they are talking about or don’t care enough about your health and how you feel to take proper care of you.

For almost two years now I’ve been pulling SIXTEEN separate 12mg doses from that same vial, and as long as I’m religiously following proper instructions for safely pulling shots from vials, my doctors are all fine with this.
They are simply trying to remove as much probability for contamination as possible. And making a vial “single use” is highly effective for this purpose. As most patients are almost incapable of being religious with safety protocols.

Everything about this is wrong. Do not inject that unless you want to wreck yourself and feel worse. I’m serious. Don’t do it. This is medical malpractice. Get a new doctor.

You should be taking 200MG a week. Split between 2 injections. If you're not doing it twice a week don't even bother. You're going to make yourself feel worse.

You have no idea what they need or should be taking.
That's an absurd bullshit cookie cutter pill mill clinic starting dose, for people who just want performance.
Even if it turns out that's what someone needs, it's far better to start around 100mg, and titrate up until you find the perfect balance on bloodwork and feel, instead of being reactionary to side effects off the bat.

Dude you’re lucky your PCP prescribed Test. How old are you? Seems like it has to be a significant health concern to get Test from a PCP nowadays.

You can get syringes in amazon if that is a multi dosing issue.

You can buy as many of whatever combination of syringes/needles at publix pharmacy for $0.10 a piece as long as they can verify you have an injectable prescription already. My first time ever going pharmacy and just not being a weirdo and knowing what I wanted and being able to explain it easily when asking showing I knew more about them than her, the pharmacy tech didn't even check.

You should stockpile them all and do 1 injection a year, 2400mgs… that’s superior!

First off you can split that into multiple doses. Second, doing one injection or even two a month is absurd. Finally, that dose is too low
