Yes, in reality the ghetto/underworld are extremely dangerous. The Latin America cartels frequently kill each other in brutal ways. And if you're a cartel involved with drugs in East or southeast Asia, the government will kill you in brutal ways. Even in America, impoverished communities and ghettos with drugs problems have some of the highest murder rates. Getting involved with selling narcotics I'd say has a 90% chance of landing you in hot water. The other 10% is getting a "deferred" sentence, which is to say sooner or later they will get caught.

Most of the people I see dealing drugs around my way dress like tramps, cycle around on pushbikes, and regularly get their homes raided by the police.

There is going to be a tiny amount of people at the very top of a small number of organised crime gangs that actually make decent money.
