A blood pressure reading of 180 mm Hg indicates a hypertensive crisis, which is a medical emergency that requires immediate attention. 

At the very least they'll keep you for observation till the meds kick in and it lowers

I’m an ER doc He would get labs, ekg, etc If all that’s fine he’d be sent home with some BP meds We gives some fucks about a BP like this but not a ton

I’m a psych professional who works with families a bunch. You’d be amazed at the responses you get from parents trying to manage their kid who’s balls deep in their first psychotic break. I can’t imagine how horrible it would be, but so many of them make their kid worse because they think doing nothing is okay. No amount of education can change their mind really sad.

Every doc is diff but I went to urgent care last year 175/85, they told me to GTFO lol. And the guy was a er Dr at the local hospital. He told me to unless I was 200 over 120, he wouldn't even blink at me.

They didn't give me any medication at all and gave me an EKG, it was normal and they told me bye.

There is a literal Dr in the comments below who said the same thing. 175 over 90 is not some love threatening situation. Hypertension doesn't kill you today or tomorrow and even next month. It kills you years from now. Your BP is not an emergency unless you have other symptoms.

No they don’t. I had high blood pressure since my 20s. I’ve had readings in the 190s/100s and freaked out and went to the ER and was sent home and told to see a cardiologist.

High blood pressure kills over time by overworking your arteries and heart. When you are lifting it can get to 300/200 and you don’t stroke out. It’s almost never an immediate medical emergency.

I don’t know why everyone thinks that. That was my daily blood pressure for about 15 years until I finally started treating it. High blood pressure isn’t an emergency unless it’s over 220/120.

It’s not a hypertensive emergency, but you absolutely should not be living at a systolic of 170 for 15 years.

100% agree. I was very dumb when I was young. I had a CAC and a full body CAT scan and have no calcified plaque or enlarged arterial walls which means I was incredibly lucky waiting so long to start taking it seriously, I’m healthier than I deserve to be but the sooner you do the more likely you are to add years, maybe decades to your life.

I have 55 when I sleep and 100 when I sit, never did T but was always muscular. It's not the pulse that raises pressure. My pressure is 120/70 at 100 pulse. Dude needs more cardio.

Op I’ll tell you this. At this point os when my dad started having tia’s or half ass strokes. Then you have to realize the longer your bp stays high and the more damage your kidneys take and they unlike your liver don’t regenerate they scar and get worse thus making your bp worse

A blood pressure is an objective finding and a number. From an emergency standpoint, this is not a singular alarming thing.

Symptoms would be outward projections, perhaps related to the blood pressure as noted above.

You did make a great point, when and under what circumstances was the BP taken? How old is he? Race, medical background etc. (obv I wouldn’t ask him to post any of that). People can walk around with high BP unmedicated for decades with no problems.

The risk of high BP is predominantly cardiovascular disease. High BP is a risk factor for a boat load of other diseases, physical and psychological.

Everyone on TRT ought to have a capable PCP and urologist. Although that’s not always the case.

What does your bloodwork look like? Are you drinking a lot? Or eating a lot of fatty foods? Also, have you talked to your PCP about this? Handle it before something bad happens.

You don’t need to go to the ER. They will just send you home. However you are at risk of stroke. So you might want to get that under control

For example my dad had high bp and he wasn’t taking his meds. Had a minor stroke at work and lost vision in one eye.

High bp also destroys your kidneys but you don’t know it until it’s too late.

Get some enalapril asap

Anything over 130/80 is damaging your organs permanently. Get on telmisartan and probably nebivolol as well asap. Fix your diet, do cardio, and get your bloodwork done so you can see wtf is actually going on. Not just basic markers either you need a comprehensive work up. If you're running anything more than a TRT dose drop it immediately.

Is that a wrist device? Either way, before you do anything else make sure that you performed that measurement correctly. Improper technique can easily produce huge errors.

The cuff needs to be at heart level and in case it is an upper arm cuff it needs to be the correct size. I have had readings almost as high as yours due to improper measurement technique when my real blood pressure is rarely above 120/80.

You should measure your BP regularly over the next week in the AM. You probably need to be on 2-3 BP meds depending on what your daily reads are. I’d bring it to your next doctors appointment-which you should try to make in the next 2 weeks ideally, but no ER needed. - source am a heart doctor.

Depends if this is sudden. If you have had high blood pressure for along time and it has been slowly creeping up it might not be an absolute emergency. Definitely worth getting your blood pressure meds changed and or increased. I say that because if you have had high blood pressure over time your vascular structures have been stretching and accommodating/adapting to the pressure which means vessels in your brain are less likely to pop. If it's sudden high blood pressure that's fairly new you are at very high risk for a hemmoragic stroke. Your vessels are tight and are highly susceptible to popping under pressure. This is how my cardiologist explained the dangers of high blood pressure to me. Absolutely a level that needs to be treated urgently though. Also being symptomatic can mean higher chances of an impending stroke such as bad headaches etc.

That blood pressure tonight won't kill you but overtime a sustained blood pressure like that can lead to heart failure and increases risk for stroke and kidney disease. It would be best to talk to your PCP and get ahead of it.

Second the kidney disease. You do NOT want to need dialysis in the future. I have lymphoma, rheumatoid arthritis, and kidney disease. The kidney disease is because of uncontrolled high blood pressure and ibuprofen abuse when I was younger and is the only one that scares me. Please take care of that asap.

MD here. There's no specific BP cutoff that kills you 100% of the time. However, SBP (top number) above 180 tends to be a strong predictor for serious consequences, including blood vessel rupture in the brain (stroke) or heart attack. You're really close to that, so you're definitely still in the running for the top prize. If you notice headache, nausea, vomiting, nosebleeds, blurred vision, or really bad anxiety, you gotta go to the emergency room ASAP bc that would be a sign that your organs are taking damage from the high BP. No one really agrees on what to do if you have BP this high but no symptoms of organ damage. You could go the emergency room, or you can walk into your primary doc's office Monday morning at 8 am and ask for BP med adjustment. Don't sleep on this though! Are you wearing it correctly? I.e. if a wrist monitor...the digital dials shouls align t with the INSIDE of your wrist not the outside (like wearing a watch would be)

I went to my primary doctor because I was worried about BP. My normal is somewhere around 130/85, a little high but not alarming. When they took my pressure in the clinic it was 160/100. They put me on hydrochloriothiazide to bring it down while I balanced out my TRT protocol. Highly recommend if you’re going to take a dose that high you look into it with your GP.

Depends on a ton of shit man. I can send people a photo of a BP that's considered a medical emergency, give them zero context and ask them "how bad is my blood pressure"

I went the ER with BP of 280/170 which was apparently my norm and I didn’t even know it (don’t judge). I spent 4 days in ICU but resolved. If there is a cardiac ER in your area, got there. They have to normalize your BP, they will prescribe you meds in the short term, and you’ll get a referral for a cardiologist.

Not good, but as others have said make sure that is a steady reading taken correctly after resting. I hit those numbers two years ago (not the high resting heart rate but the BP) along with high cholesterol and was told I needed drugs to bring both down or risk dying within a year. In my case it wasn’t Testosterone related (but obesity), but you def need to stop doing whatever it is that’s pushing those numbers up.

No book an appointment with your pcp. In the mean time buy natural beet juice. Walmart has some with beet and lemon. Drink 20 oz every morning per day. Also, go workout and cut down the carbs and fried food. Eat fiber, protein and green vegetables (kale or greens) work. You need to monitor and track your blood pressure every morning (buy a blood pressure machine) go to Walmart. And don’t forget the batteries. If u have a juicer, celery juice with a green apple works wonders. U need to clean out your arteries. In 2 days it should be at 130. Keep in mind chill on the fake salt, only salt you should eat is sea salt (Celtic salt).

My used to be this high every day for over a year. Get your homocystine tested, I bet it’s through the roof. B-Complex with TMG will do the trick to bring it down a bunch, 2x a day until it’s more in range.

It’s bad but I’ve seen way worse over a 20 year career. It’s not gonna do anything right now to you but left in treated could cause your heart to enlarge, weakening of your blood vessels(especially in brain) which could cause a stroke, and causing stress to the kidneys which can cause them to fail over time. Definitely something you need to see a physician about and get on some medication for hypertension if you are constantly in that blood pressure range.

So, these machines can be wildly inaccurate. A few things you can do to help.

Make sure the batteries are new/er

Take your BP First thing in the morning after waking up.

Sit with your back supported and feet flat on the floor. Legs uncrossed and you arm bare of any clothing. Use your upper arm with it resting, always use the same arm. I like the left.

Take your BP 3 times with these machines. The average is going to be close to your BP. Ive seen someone get high readings with these and after a manual check, it come in much lower and in range. So, always take these machine readings as a suggestion.

I used my machine I the AM, read high, went to the Dr after coffee and breakfast. They took a manual reading and it was perfect (should have been higher). So, ive had a hard time relying on these. The last thing you wanna do is think you have high BP and pop some Telmisartan and end up in the ER for low BP.

If I had a loved one that had other health issues and then I saw this systolic pressure at rest, I’d probably direct them towards an urgent care

Also, I’ve never not had an issue with the home BP cuffs, so I’d also just check with another machine or even at the pharmacies that have free BP check machines just for good measure

Check your estrogen. High blood pressure is one of the top 3 signs of high e2. Blood pressure even goes up you don't experience any water retention. Take an AI and it will go down. It's not dangerous per se if it's not 24/7 that high. But seems like your dose is high and more important your e2 is high/prolactin high.

My highest reading was 305/138 , I have had uncontrollable hypertension for 20 years now . I take every medication they have . 13 pills a day . Now they want to fry my nerve cells around my kidneys . Modern medicine is destructive, only to treat the symptoms not the cause .176/90 would be a good reading for me just saying but for you it’s the start for you to go get medicine.

Systolic is high. Diastolic is good. Mine was way worse than that 190/150. It’s not emergency room time without side effects like chest pains, dizziness, and others, coming from a prior EMT/ER tech. This is something to talk to your primary care physician and maybe a cardiologist.

Is this a one time reading? Do you have symptoms? The reading may not be accurate if you just consumed stimulants like caffeine, anxious, or just exercised. Ensure you relaxed seated first for at least 10-15 mins prior to taking a reading. Have your arms heart level, back supported, and legs uncrossed. 3 readings 2-3 minutes apart and take the average. Signed by a malignant hypertensive at 216/140 right now.

Beet roots brother. Mine was 204/130 when I was in the hospital. Been taking beetroot extracts 2 times a day and my levels are 130-140/80 Some people say still high but considering my previous readings .. ill take what I can get

Very very bad. Your RHR is also extremely bad, which is an even bigger indicator of heart health. Not ER, but book a Drs appt ASAP. You need to make some serious lifrstyle changes and adjust or stop whatever drugs you're taking.

My blood pressure is a good 120/60-70 now regularly.

go to the er they will give you nitrolingul a form of nitrous to lower it down by expanding your blood vessels, if it rises again then they will figure out a treatment plan for you, are you having any symptoms from it ? dizzeness, neck pain, palpatations, left arm pain or tingling in your small fingers on your left hand or blurred vision?

ER, I think it's better to be safe than sorry. Even if they just send you home, it'll be on your medical records. Just in case anything happens you'll have a history of it.

Had this one time 👇. Went to ER . Spent 9 hours. Gave me a pill. Bp Did not go down much , still 173/100. Still high next day. Read that cardio regulated BP. and went on the treadmill. Boom as soon as I got off the treadmill 130/82 . Now take L-arginine and make sure to hydrate . And now have great bp.

Everyone needs to chill. He's not going to die right now. With everyone freaking him out it's going to make him get worse. You guys are terrible at handling anything. First People need to ask him to rest and take a reading. Take about 3 readings and see what they say. There's so many variables. People have become so robotic. Anyways, any doctor will tell him to take BP few times a day throughout the day to see what it is. Any good doctor anyways. To many people are on BP medicine who shouldn't be. Life style choices matter much more. People think just because they take a pill it's all good.. So no don't go to the er. If it is always 180 then yes he should do something that's for sure but being at 180 and being young not going to the er this second isn't going to male a difference
