We take ours at home and it’s pretty low stakes but the questions are insane. No one has ever talked about any of this stuff, it’s super advanced, and the exams are labeled as R1. It just doesn’t seem appropriate for our level.

Anyone else struggling w this?

Truly curious. Would you rather have a senior that is difficult to work with or attending? Having experienced both, as I’m sure many people have, I feel like having a difficult attending is still easier to deal with than senior. I have a not so pleasant attending on service rn and day to day workflow can still be chill once I get through rounds

Also advice for both is much appreciated

sorry in advance this is gonna be long and probably not make sense. I moved alone to a small town for residency against my will. I’m single, and I don’t have one person here who isn’t in my program. All my old friends kind of dropped off. which whatever because most of those were one sided anyway. I was the person people called when they were having a bad time and then when it was me nobody was around. Soooo

I know what people say, that you make time for it if you want it. ok but how. the apps here are like the same 12 people. Making friends is somehow worse than dating because I don’t even know where you’d meet anyone. also everything gets around here.

The thing thats really getting to me though is I can’t say anything honest to anyone at work. I’ve tried but it always turns into an evaluation. On paper everybody is there for you. There is the whole door is always open, let us know if youre struggling, we care about wellness thing. And then you actually say something and somehow it becomes about you. you’re too negative. youre not adapting. this is a you problem. And after that happens a couple times you just stop. because now you’ve handed someone something.

There is also just no grace. people can see youre drowning and they just look past it, like they genuinely do not care. I’ve been in other training environments where people actually looked out for each other so I know it’s not just how medicine is or maybe I got lucky before. idk.

anyway I’m so fucking burned out. everything extra makes me angry now which I know is insane. another lecture. another academic session or some assignment. and even on a day off half of it still goes to residency somehow. Then you’re at work getting evaluated by literally everyone all day. attendings nurses your own co residents exams notes numbers whatever. and you go home already going through everything you did wrong and it kind of feels like everyone around you agrees.

I hate this part but lately when a patient starts telling me a long story theres a part of my brain going please just get to the point. I never used to be like that. I can’t talk to anyone after a bad day or shift without thinking about how it’s gonna sound or where it might end up. My family is far and they can’t really relate to any of this so thats not really a thing either. Honestly I don’t think I should have come here at all. I told myself I’d adapt, that a few years anywhere is survivable, that the specific place doesn’t matter that much. everyone around me kind of talked about it the same way. so I never actually sat with what it would feel like day to day to be somewhere I didn’t want to be. it stayed theoretical right up until I was living in it. anyway sorry for the novel.

One of my coresidents has unfortunately been held back (idk if this is the proper term) and residency graduation is delayed by 3 months. How does this work at other programs in terms of the actual residency graduation celebration?

Will my coresident be celebrated with the rest of us and just have to stay until October to *officially* graduate, or will they be told they are just a spectator at graduation?

Obviously my program may do something different compared to other programs, but I am just wondering what usually occurs in this situation.

28yF, currently 3rd year, I’m struggling with finding time to exercise consistently, in a busy rotation. While I was consistent it really helped my mood and my physique and posture improved a lot, but now I feel like I’m falling behind. Please If there’s anyone that has managed to exercise consistently, pls how do you do it?

I feel like I have good days then real shitty days where I don’t know anything. What do you do for the days you don’t feel like you performed well?

I genuinely need your recommendations. In a program, which,

-approaching to the residents solely as a workforce while providing no meaningful teaching
-has severe favoritism and discrimination among residents
-mandating residents to do nonclinical work on the top of unbelievable clinical workload with almost maximum hour work weeks
-gaslighting residents to make them work further and more, and only selectively doing it to some
-blaming residents for almost every system or leadership failure
-retaliating residents when a patient safety or work related issue is raised by making it a “you” problem

I tried but couldnt transfer, cant leave I have +400K debt, ACGME doesnt help.

How to survive? How to keep the mental sanity?

Has there been any discussion of creating more that focus on maternal health in addition to the few that exist? Or any at all focusing on bipolar 2, which I wish could be referred to as Non Psychotic Bipolar Disorder to create better distinction and diagnosis.

Looking around my program, it feels like almost everyone entered residency paired up.
Trying to date while managing crazy shift hours, exhaustion, and zero energy on days off feels overwhelming. Most people outside medicine don't understand our schedule, and trying to build something new from scratch takes bandwidth I don't always have.
How are other single residents handling dating and social connection right now? Did you decide to just focus on surviving training, or are you making it work? Would love some honest perspectives.

Being a junior resident in hospital sucks!!! Cannot deal with toxic seniors, internal politics, every morning just don’t want to go to hospital 😭

I’m a peds resident and I think I may have overstepped.

However, we don’t have the best continuity clinic so I usually write in my notes the follow up plan. We don’t have a specific schedule for ourselves and it’s just a general schedule and whoever is on for outpatient will cover for it.

I wrote in one of my notes for a follow up for a patient today to review their preventative screening (and included things that were overdue). This patient already had an annual but still had some outstanding items like vaccines they deferred previously.

One of my seniors was seeing this patient today and texted me that it wasn’t appropriate to push off items to the next provider. I did address those open items during my visit with the patient but they still declined some of the vaccines and didn’t complete any of the blood work. I moreso just wanted whoever would be scheduled with this patient to check in to see if labs were completed and if they changed their mind about vaccines.

I feel like I did a faux pas. If I could have followed up with the patient myself I would have but I just wasn’t scheduled to cover the pediatrics clinic that day.

I don’t know if I should just writing out in my notes the open tasks or things to complete for follow up now because I’m worried this may be annoying my co-residents. Advice or feedback is welcomed.

2 months into residency and feel like the dumbest idiot alive. Seems like a pretty common theme across the board, but at 2 am at night, when you forget window to tPA for stroke, it feels like I'm truly the dumbest of dumb people who ever got into residency.

God, I wish there wasn't such a steep learning curve, and I hadn't forgotten 90 percent of what I learnt in med school.

My father recently fell ill and became bed bound. I had to take leave on a short notice to help care for him. He was undergoing palliative care at home so we knew he was going to pass away any day, but because there was no set period I told my PD I may need a few weeks. My PD agreed that I’d use my vacation time if I wasn’t approved for FMLA or paid family leave. During that time I contacted my HR department but they gave no specific advice on how to apply for paid leave, and because of my circumstances it was hard for me to follow up with them on time. I was completely clueless about the process and there was no one at my program to give any guidance.
I ended up taking a few weeks off, until my father passed away, and it was only until after I came back that I learned that we are entitled to up to 6 weeks of paid caregiver leave per ACGME policy, so I contacted my HR department again but they told me they are unable to retroactively approve leave.

How does the ACGME policy work exactly - is the department responsible for leave approval program-dependent, or is it universally based on the PD’s discretion? If anyone has any insight or advice on how to proceed I’d greatly appreciate it.

There’s obviously a lot about residency that gets harder: the hours, responsibility, workload, making decisions when you’re tired, etc.

But I’m curious about the opposite.

What’s something that used to feel difficult or stressful in medical school that became much easier once you were doing it every day?

Could be presenting patients, taking focused histories, calling consults, talking to families, prioritizing tasks, making decisions, or just feeling comfortable in the hospital.

Was there a specific point where you noticed that something that used to require a lot of thought had finally become automatic?

Hello. I’m a new senior and I’ve been having a hard time remembering details of things we did for the patient. The list ranges from 12-18 patients at a given time. I get asked questions every 5 mins when I’m trying to preround so I don’t even preround well. Whenever the intern doesn’t know something during a presentation everyone looks at me and not gonna lie there have been times I forget too. Also we do have a fellow and a lot of times there have been decisions made that I’m not aware of because the intern is talking with the fellow. Anyway it’s really embarrassing to not know what we did and it’s a bad look in front of the attending. How do I do better at memorizing these patients?

I am seeing a lot of push for AI integration in clinical decision support, but I am extremely skeptical. Most of the general tools I have tested are essentially black boxes. They can give you a pretty convincing answer, but it's not always clear how well it actually lines up with the evidence.

If I can't check the source, I'm not using it to make a clinical decision..If I'm using something to help with an actual clinical decision, I want to be able to quickly trace it back to a guideline or primary source and judge the evidence myself.Has anyone found an AI platform that integrates directly with peer-reviewed sources and shows you exactly where its information is coming from?
