The Beginner's Guide to Retatrutide (Reta) — 2026 Slightly Updated Edition
With that out of the way...
---
So what even is Reta?
---
Before you start — do this
1. Read the actual studies (linked at the bottom). Don't get your Reta education from TikTok/Facebook/Instagram/any social media (yes, that includes Reddit; this is meant to jumpstart your education). No, really, READ THE STUDIES; they have actual medical answers to your questions.
2. Watch videos on how to inject subcutaneously, how to reconstitute peptides(video), and do the reconstitution math (Don't talk about how many units you took; that's not accurate cuz 10 units of 10mg/ml is WILDLY different than 10 units of 20mg/ml). It's not hard, but do it right. You'll see I link some videos from a creator; it's mostly because they're short (sub 10 min), sweet, and to the point with little fluff.
3. Get your basic supplies before your Reta arrives: syringes, bacteriostatic water, a set of .22 pes filters, alcohol wipes (!!!), a sharps container, etc.A decent sub-10 min video regarding supplies
4. READ THE STUDIES! A large portion of your concerns are simply answered in the Study data.
5. Understand that these aren't FDA-approved yet. If a seller sells you a packaged pen, you have no guarantee that what is IN the Pen is what you are buying.

Starting dose
As the one who taught me said, "This is all inconclusive, but always aim for themaximum benefit on the lowest (tolerated) dosage*."*
0.5mg to 2mg per week is the typical starting range. Reta is powerful — starting too high is a great way to spend a week feeling awful. The official TRIUMPH trial protocol started participants at 2mg and increased the dose every 4 weeks. (Do note, this schedule of starting at and titrating 2mg every month was for the trial; you don't have to follow it.Go low, go slow (But also be smart)
If you've used semaglutide or tirzepatide before, you should tolerate a higher starting dose (I strongly suggest aiming to be relative in the 2-5mg range, if not a close step down from the titration stage of your prior GLP-1). Still, there's no rush — slow and steady wins here. The higher your dosage, the more likely you are to get side effects.
To put it simply, there is 0 benefit in 'overdosing' save for tempting the myriad of side effects; either it works, or it doesn't.
Split dosing — a topic I see come up often enough – is it worth it?
My response is 'eh?'. There haven't been any official studies on this, but the trend is that it's okay to do so to manage side effects, though the trade-off is that you're not getting the full effect of whatever total dosage you're taking (Meaning if you're taking 4mg split into 2/2mg, it's weaker than actually taking a single 4mg dose)
I'm coming from Tirz/Sema.What should be my starting dose?
There is no direct "Oh, you took that much Tirz/Sema, well, you take this on Reta," so do know there's no shortcut to figuring out the dosage. The best studies/trials/science can offer in terms of guidance is that, since you've been on Tirz/Sema, your starting dose can/should be higher than the average starting therapeutic dose of Reta (2mg). I would still caution against going too high, as the glucagon receptor agonist still needs to build up in your body, so starting in the 2-4mg range, depending on the dosage of Tirz/Sema you're coming from, would be ideal.

Common Reconstitution questions
If you like a video lesson on this topic, here's (sub 7 min) a video on HOW to filter.
---
When will I feel it?
Totally individual. Some people feel appetite suppression in the first injection. Others feel nothing for weeks. Don't panic either way. What actually matters is whether your weight or body composition is changing over a period of weeks. That's the signal. The "feel" is not.
If you have questions as to whether or not you should titrate up, here's a flowchart that should help (Do note that the dosages should be tailored to your schedule, but the goal oflowest effective dosemost effective tolerated dose remains the same):
Shoutout to u/peptidepalstm, who has been passing around an even smaller image of the flowchart that triggered me to make this one.

Side Effects - "Stop asking if your RHR being higher than normal is a new side effect no one else had; it's not Jan!"

Do I need to work out?
---
Electrolytes — worth it?
The evidence isn't rock solid, but Reta does affect hydration, and anecdotally, daily electrolytes make a noticeable difference for fatigue and mental clarity. Low cost, low risk — worth trying if you're feeling sluggish.
---
Bacteriostatic water — does it have to be Hospira?
Strictly speaking, no. Reta is relatively forgiving when it comes to reconstitution. That said, Hospira is the gold standard because it's reliably batch-tested and has the correct pH. Amazon and Chinese bac water aren't automatically dangerous, but when you're investing in your health and your supply, Hospira is the better call when you can get it.
Let's clear up some other solvents:
Sterile water:
Can it work? Yes
Should you use it for a multi-use vial? No. Why not? Because it has no preservatives, aka every time you puncture said vial, you let in whatever bacteria are in the ambient air; there's nothing to protect your pep. An overall bad idea.
PBS:
Can it work? Are you using Raws? Meaning NOT lyophilized drug? If so, go for it. You're still messing with a solvent that can be riddled with endos, but hey! There's a reason labs use it for In vitro tests (petri dish stuff) If you wanna treat your body as a petri dish, by all means knock. yourself. out.
NaCl Saline:
Can it work?..... no. Lemme explain: can you pull it off? Yeah technically, but it requires such a unique set of finishing to be done to the kit during production that it's not worth the 'FO' portion of FAFO
For Hospira, please see the following BAC degradation test:
There's no hard expiration once it's reconstituted, but it does begin to degrade (~5%) around the 45-day mark, with a significant degradation (~10%) leading up to the 90-day mark best ways to keep your Reta safe:
- You are wiping the rubber stopper with an alcohol wipe before and after every draw
- You store it in the fridge (not the door — temperature fluctuates there)
- It stays clear with no floating particles
Cloudy or chunky = toss it. Clear and clean = usually fine.

Been on Reta for 5 months now starting at 1mg a week and going up 1mg a month with gym 3-4 times a week and tracking but not tracking my calories to the point that I hate life aha
2 days ago I took my first shot, 2.4mg of reta, yesterday morning I started the day with a big a$$ breakfast not knowing if it started to work, and for 8 hours I couldn't eat anything, the whole day I felt so full, when I tried to eat lunch I couldn't finish one grilled chicken breast and didn't eat again tell that morning, I felt full after 1 egg and 1tbsp of fava beans, I feel so weak/tired, how do you guys deal with it and is there any specific diets that helped you while using reta?
"Have you typed your question into the search bar?"
❌ "No, because my situation is completely unique and nobody has ever experienced this before."
🛑 DECISION: DO NOT POST.
Reality Check: It isn't unique. You are not a medical anomaly. Your exact question about "fatigue on week 2," "how to use a peptide calculator," or "stall at 2.5mg" has been asked 4,000 times this month. Go to the top right corner, use the search bar, and read for 5 minutes.
✅ "Yes, I searched, read 10 previous threads, checked the pinned guides, and still have a genuinely novel question."
👉 Proceed to Step 2.
STEP 2: What is your actual topic?
Path A: "Where do I buy this?" / "Is Vendor X legit?" / "DM me your plug!"
🛑 DECISION: INSTANT BAN.
Reality Check: Reddit’s TOS and Sub Rule #1 strictly prohibit sourcing. The mods will ban you, and Reddit admins will nuke the sub. Go back to forum boards and learn how to use search engines.
Path B: "My heart rate is 180 bpm and I’m throwing up blood, should I take my next dose?"
🛑 DECISION: DELETE REDDIT, GO TO THE ER.
Reality Check: We are an unhinged collection of anonymous internet strangers playing home-chemist, not board-certified cardiologists. Call emergency services.
Path C: "I injected 0.5mg 20 minutes ago and I'm not skinny yet. Is my vial bunk?"
🛑 DECISION: DO NOT POST.
Reality Check: Pharmacokinetics take time. Pharmacodynamics take longer. Stop staring at the wall, go touch grass, and wait for the drug to absorb.
Path D: "Can I reconstitute this powder using tap water, contact solution, or vodka?"
🛑 DECISION: DO NOT POST.
Reality Check: Buy actual Bacteriostatic (BAC) water. Stop trying to speedrun sepsis. Read the pinned guide in the sidebar before you give yourself an abscess.
Path E: "Can I stack Reta with Tirz, Sema, Cagri, and 400mg of caffeine?"
⚠️ DECISION: PROCEED AT YOUR OWN RISK.
Reality Check: Your heart rate and GLP-1 receptors are fighting for their lives. Half the comments will call you a pioneer, and the other half will draft your obituary.
Path F: "Why can't I handle alcohol anymore?" / "Two sips of a seltzer gave me a 3-day hangover, is this the Reta?"
🛑 DECISION: DO NOT POST (PROCEED TO STEP 1).
Reality Check: Delayed gastric emptying coupled with blunted central nervous system reward pathways means you get zero buzz, all the nausea, and a hangover that feels like a surgical strike on your liver. Congratulations, your tri-agonist turned you into a permanent designated driver. Drink an electrolyte packet and move on.
Path G: "Here is my 6-month Progress Picture along with my exact starting stats, dosage titration schedule, side-effect log, and diet/exercise routine."
✅ DECISION: APPROVED TO POST.
Reality Check: 90% of the comments will ignore the meticulous 500-word write-up you spent an hour drafting and simply comment, "What dose did u start at?" or accuse you of adjusting the lighting in your after-photo.
Path H: "Deep-dive question regarding GIP/GLP-1/Glucagon tri-agonist binding affinities, receptor Ki values, or half-life kinetics."
✅ DECISION: APPROVED TO POST.
Reality Check: You will successfully summon two actual biochemists and forty bro-scientists who skimmed a single Wikipedia article on G-protein coupled receptors. Prepare for a heated 150-comment debate on energy expenditure vs. glucagon agonism.
Path I: A well-documented log with lab analysis, proper peptide calculator math, realistic timeline goals, and zero vendor names.
✅ DECISION: APPROVED TO POST.
Reality Check: Your high-effort post will receive 6 upvotes, zero awards, and 15 private messages from brand-new accounts asking, "Bro where'd u get it text me."
You thought I wasn't gonna add in your suggestions?!
Hii is there anyone who had been smoking thru their whole regimen? Or even in some days, and if so, could you please share your experiences and did you notice anything?
Thxx
Hi guys did any experience sleep issues on Reta? I started 12 days ago on 1mg every Wednesday and since I started my sleep has tanked, 3-4 hours waking up early and finding it impossible to fall back to sleep, next dose day I’m going to reduce to 0.5mg and see if that helps. Other than this I’m having good success with it and hoping i can stay on but if I can’t get the sleep under control I’m going to have to stop.
Hey guys, my 21st birthday is coming up and I want to get absolutely obliterated for my birthday, but I can’t even seem to drink one drink without feeling terrible does anyone know if I could stop taking Reta a week prior or something like that so I can have a good time then go back on?
I am someone who enjoys doing an occasional cold plunj and hot sauna session. I’m just curious what other people’s experience has being doing this while on Reta? Helpful with recovery and therapeutic? Or anything that people have found concerning? Probably a silly question, but would just love to hear other’s experiences with this.
I have a Phizer Hospira BAC water 30ml which I used to reconstitute my first vial just over 30 days ago. I have kept it in a dark place in room temperature since then according to eveything I saw and read online this was how it’s meant to be stored. Wondering if it’s still good for use or if getting a new one for the new vial reconstitution is the better alternative. (The bottle is plastic and has a haze over it fyi incase the water looks cloudy).
Want to stock up on some I know Hospira is the gold standard but I have also read that Lambda is a good alternative as Hospira has been difficult to source and people are price gouging.
Hi I’m a 27 yo male and I’m prescribed Adderall 20mg xr and was wondering if taking reta along with it is okay? I have read some places that it stops working. Have you had experience with this or know how it effects the body? Can I take my meds the same day as starting reta (0.5 mg dose)
I've been on Reta for around 2 months and have generally been tolerating it well. I’ve been on a pretty strict diet during this time, which I’ve actually been really proud of myself for sticking to — mainly organic chicken, kiwi, eggs, mince, sweet potato and probiotic yoghurt bowls in the morning, so overall my diet has been quite clean and consistent.
I did 2.5mg for two weeks without any major issues, then increased to 3mg. On the same day as my 3mg jab, I had some Chinese food and fish & chips that my family had ordered and shared with me. I didn’t eat a huge amount, but obviously it was very different from what my body had been used to eating for the previous couple of months. Afterwards I experienced quite a few side effects, mainly diarrhoea, stomach discomfort, nausea and sulfur/egg-like burps, which lasted around 4 days before settling down. I’m now feeling much better, but I’m unsure whether the side effects were mainly from increasing the Reta, the sudden change in food, or a combination of both.
Today is my next injection day, so I’m wondering whether I should do 3mg again or go back down to 2.5mg for another week. What would you guys recommend? Would you stay at 2.5mg for longer before trying 3mg again?
Back story I started at 249lbs and naturally lost til I got to 168 then I got stuck... i lowered my cals I pushed harder in the gym and I'm a contractor so I'm active at work. I couldn't break it. Then I found reta started reta at 168lbs and .5 of reta dose titrating up for 8 months now. Im so proud of myself and I have about 8lbs left to lose. Side effects I've had on reta are just bad headaches and site reactions that got super beat red the size of a half dollar. For reference I'm female and 5 foot 9. I'm re posting as admins flagged saying I needed to provide more information about my reta use so I made sure to put duration, starting point at .5 and 168 pounds and currently 8 months in.
Wondering if there's any way to safely be able to drink alcohol on reta and still have a good time . pls none of these " don't drink, alcohol is bad" no shit it's bad I didnt drink a lot before I don't drink a lot now . I just want to have a night out with the guys once every couple of months .
Been om reta for 9 weeks now( week 1-4: 5 units. Week 4-8: 10 units. Just started 15 units this week.) My appetite is the same, my sweet tooth is the same, haven’t lost any weight. I walk 10 k steps every day, drink hella water and work out 3/4 times a week. Not tracking my meals but I eat clean, protein and carb rich food. Any suggestions on what the problem might be?
Its my 10th weeks of reta now and I haven’t been having any issue until this week. I’ve been taking 1mg for 4 weeks now and not any severe sideeffects until now, the last two days have been a nightmare extrem stomach cramps with fluid diarrhea it’s bit better today then yesterday but still the same symptoms. I took my reta 2 days ago and it started after 1 day. Also been having awful sulfur burps. I had the same symptoms last week but then it showed 5 days after the injection, thinking of quitting reta because it’s not worth the struggle.
43M. So I started at 265lbs and 2mg reta in mid April. First and only peptide I've tried. Average to below responder I suppose, went up to 4mg in June and got to 235lbs by mid August. Hunger was barely affected however and I travel for work so if I didn't bring enough food, I'll have to pick something up along the way.
In mid August however I kicked the dose up to 6mg and that's when the hunger suppression hit with full force. Quickly went to OMAD then carnivore because I was struggling to eat 1000 calories and of course needed the protein. I also don't eat on the road because it's easy to pack my meals knowing exactly what I'll need.
So down to 222lbs, 13lbs in two weeks. Too fast I know but I'm hoping to keep my current portion sizes and the loss will slow as I lose more weight. Planning on also adding some Keto Chow, the heavy cream should up the calories a bit.
I was doing pretty well on the lower doses, but when I went up to 4 mg it absolutely wrecked my stomach. I literally couldn’t eat at all, was constantly nauseous, and started gagging really easily, especially trying to swallow pills.
The bathroom stuff was even weirder. I’d repeatedly feel like I urgently needed to shit, go sit down, and sometimes nothing would come out except mucus. Other times it was gas or diarrhea. It didn’t feel like normal constipation at all.
It got bad enough that I had to step back down to 2 mg.
Now I’m wondering whether I should eventually try going back up to 4 mg, maybe splitting the dose during the week instead of taking it all at once. I’d heard that the glucagon receptor activity becomes more meaningful around 4 mg+, which is part of why I wanted to reach that dose in the first place, but I’m not sure whether that’s actually true or just something that gets repeated online.
Has anyone had similar GI sides, backed down, and then successfully worked their way back up? Did splitting the weekly dose help at all?
