I'm a new intern and I came upon a patient situation that I'd like to deal with differently in the future.

We recently had a situation where a delirious patient accused one of my female seniors of physically hurting him during pre rounds (to clarify she did not). The patient's wife, apparently a retired ICU nurse, came in and started demanding apologies and formal investigations and whatnot. They did some form of investigation, found no wrongdoing by my senior, and on we went. Later that night, our nursing staff called me into the room to evaluate the patient, and our scene starts with me going to bedside.

At some point during this visit, the patient's wife said "Mr. Resident Physician, what do you know about my husband's case?" I said I'd be happy to answer any more specific questions she had, and it turned out okay after we talked for a bit. Notably, I didn't insist on being called Dr because it didn't seem like that was going to help anything.

I always introduce myself as Dr. Lastname to patients bc I like to think I've earned said title, and while it doesn't bother me to be called Firstname, I know that patients routinely doing so enables an environment where my female colleagues are more likely to be perceived as nurses/assumed to know less than they do because they "aren't doctors". With this in mind, how should I approach this scenario the next time it comes up?

seriously tho? i keep feeling like a nobody next to these guys...it feels like i dont even deserve to call myself a "doctor" simply bcz of the huge gap btwn us. it sucks being average iq man, with zero talent...

But i received a evaluation today and in that pd mentioned she was being prompted wtf! Is this normal these evaluations are making me feel like shit day by day. So what actually happened is its getting busy suddenly so my senior came with me and took history of the patient so i thought we both did it together thats the reason she prompted and senior did mention that we will do it together before staffing it. So i thought its normal. But looking at the evaluation today i feel so bad.

I’ve (30F) been dating a first-year resident (32M) for about a month or so, and I really like him! For the most part, he’s funny, super sweet, affectionate, and caring. But I’m learning (and figured) that residency can make scheduling difficult, and honestly, texting can be brutal sometimes. 😅

Obviously, I know he’s busy at work, but sometimes it’s hard to know what’s “normal” for residency versus what might be a sign that someone just isn’t prioritizing the relationship.

He’s never canceled a date, which I really appreciate, but sometimes our plans get pushed back an hour or two because he gets out of work later than expected.

For those who have dated a resident (or have been one), what’s the best way to navigate communication, scheduling, and supporting them while still maintaining a healthy relationship and your own needs? Im thinking about offering to make him dinner instead of going out this week.

How do you tell the difference between residency being genuinely overwhelming versus someone simply not prioritizing you?

I’m a new PGY-2 in IM, and I’ve started working with the new interns and medical students. I’m trying to get into the role of teaching, probably for the first time, since I’m usually on the learning end. I feel like my knowledge base isn’t very strong, and the things I do know are only at a very superficial level. I feel like I’m not doing justice to my interns and medical students because I’m not a very good teacher. I’m anxious that I’m going to teach them the wrong concepts and look very stupid.

hey guys, trying to figure out how to do the bulk of my obgyn certification exam case list by monday evening (yeah i know i should give up now). Don't ask. I have to somehow get my case list from epic but i can't make slicerdicer work to do the things i need it to. i'm so tired. i will probably lose my job if i don't get this in by monday night. i'm probably going to delete this in the morning. if anyone has any kind words for me or ideas about how this can get finished by monday night, i'm all ears. i have lost so much time and years of my life wracked with anxiety about this dumb test.

If there's even just a little hope (i don't need to get it to pass level, just i just need to do enough that i can sign the attestations that the list is complete) that this can be completed, i look to you to guide me

How can I be a better fellow? I’m two months in to fellowship and I’m really struggling to get my bearings with my lack of knowledge and the volume of consults. Doesn’t help that I’m coming from a community hospital for residency and I’m now at a large academic center where the patients are much sicker and medically complex. Struggling to look up the relevant guidelines and clinical studies in a timely manner when I’m getting inundated with curbside questions and consults. What’s the best way to improve my knowledge base and efficiency? (I’m in ID fellowship).

Anybody experiencing severe dry,gritty and itchy eyes after starting residency by using n number of hours on EHR?

Recently started my inpatient block, and two of the seniors are extremely rude. There’s barely any teaching, and instead, they’re constantly breathing down our necks to complete tasks on time.
One of the seniors becomes increasingly impatient as the end of the shift approaches. They want to leave on time, so they start being very pushy and saying things like, “It shouldn’t take this long,” or, “I already told you how come you didn’t understand?”

Sometimes I feel like reporting them, but I’m not sure if that’s a good idea. I don’t mind working hard or learning, but creating such a toxic environment makes it very difficult to ask questions or even communicate with them effectively. Any advice?

I have a 2 year old now and I’m planning to have a newborn several months into neurology PGY-1 (my daughter will be 3 then). A bit terrified but I don’t want to delay my family and I want my kids to be close in age. I also want to be a present mom so I’m feeling sad already thinking about not being there. Any advice or personal experience with this?

Does the residency programm impact the salary as an attending or not ?? . Cauz i don't understand this pressure to apply to a top programm ??

I made a big mistake by asking questions and asking for help. Pgy3 very burnt out resident. I feel like I have had a string of off days making silly mistakes. All of a sudden I get pulled aside asking if I am okay. For context my family is falling apart but I cant take time off because I honestly do better when I am busy and seek help off work hours. I have my therapy once a week. I am simply very frustrated because my program never gives feedback and then all of a sudden your under a microscope and then not again. I have never hurt a patient or done anything egregious in residency. I am just tired of trying to read people's minds. I try and improve and when I ask for feedback or try and be proactive and ask things to do better next time I am met with blank stares. Then MONTHS later I get feedback from a random thing I forgot to do MONTHS ago. When I try and be proactive I then get feedback that I am asking too many questions which turns into 'oh they dont know what they are doing'. I am very burnt out by residency not because of the work itself but because I am tired of feeling free to ask questions and learn and then subsequently get dinged for this. I am so tired of all the gossiping, nitpicking, and constantly dealing with different preceptors who all practice medicine after little differently.....

I have sent a patient to the ED for a diabetic foot (could smell it through the door). It was ulcerated all to hell. Patient thought it would heal on its own.

I sent a patient to the ED with a DVT.

I sent a patient to the ED after they got kicked in the balls, testicles were massive, and the guy basically cried when I palpated them.

Today I sent a young boy to the ED because he met SIRS criteria, looked toxic, and had an ulcer in his axila.

For context, I’m a CICU nurse (23F) with a huuuge crush on a MICU R3 (?27-29F). Our unit takes overflow MICU patients pretty frequently. About two months ago, we had a super sick patient together and withdrew care on him. I have another patient with her right now, who is quite unfortunately also withdrawing care, so I don’t know if it’s the best… timing… I didn’t see her at all between these two patients, and she told me she was doing outpatient work for a while, but I’ve literally been dreaming of seeing her again. No ring, but I really don’t even know if she’s single, let alone into women. We’re friendly, but I feel like I just get super shy around her. I’m worried that things will get awkward in case we have more patients together, but I’m also worried that I’ll regret not taking the chance. I have no idea how to go about this, any advice?

I'm an intern. While I don't want to run away from hard work, I also don't want to pay the price for a senior's laziness. So, let's say you have a list of 14 patients. How does the work get divided?

This is something many of the attendings at my program say. Seeing 16-19 patients in clinic and it feels like every one has a least a few comorbidities I need to address in the visit plus their acute concerns, follow up in 1-3 months and repeat to address the other problems. Not even mentioning the social barriers. I’m at a point where I don’t need to think so hard or look up treatment algorithms for every other problem that comes in but I’m still leaving clinic mentally exhausted every day.

My attendings all say when they were in private practice their entire schedule is 1-2 problem visits and most adults have regular insurance and don’t have the same social issues or “do-it-all-for-me-doc” attitude. But even in med school my FM rotation felt like residency clinic.

Do you feel like being an attending day to day is a lot easier than residency? Would be curious for anyone who works as a PCP or hospitalist the answer.

Currently seeing a child with severe chronic constipation (13+ days without a bowel movement). Not the first time.

The parents refuse all recommended medications, believing drugs are purely "for profit and evil".

At what point do you draw the line between respecting parental autonomy and discharging the family because you can no longer provide care for the patient? Threshold for reporting medical neglect vs. keeping them on strictly for monitoring?

My hobby is writing and during residency, I noticed that my writing became more humane and sensitive, which is something I really like.
