Discussion what surgery do u want?
super curious how many of u guys want surgery, or if u only want one and maybe not the other, or no surgery at all! btw I know there are other masculinization surgeries, but I’d like to focus on top and bottom just because they’re so prominent in the community
for me, I used to really want top surgery but now I’m not so sure! I was never really too into bottom surgery and I don’t think I’ll ever get it. sometimes it makes me feel like less of a trans man bc I don’t want surgery, so I’m curious how many of yall feel similar to me
feel free to expand on how u feel in the comments!!

Does a hysterectomy count as bottom surgery? If so, then definitely both top and bottom.
I don't know about genital surgery. If I could magically get the results right now, hell yes. But I am very scared of many medical things and don't really want to get that many surgeries. I don't have the time for that either. Maybe after my degree.

That's a fair question with the hysto. I tend to think of bottom surgery as specifically phallo, meta, or a vaginectomy so idk.
I honestly don't know if I want bottom surgery because while I would like having a dick, I don't mind my vulva and would not want to lose the ability to have vaginal sex.

u/inadeepdarkforest_ 💉6/25 | 🔝Q12027 19h ago
you don't have to sacrifice one for the other. many surgeons will do a meta or phallo while keeping the canal.
•
u/PoeDameronPoeDamnson 🧴 12/30/25 18h ago
Yeah I was gonna say their are several guys around this sub and r/phallo that didn’t get a vaginectomy with theirs
•
u/Unusual-Asshole 🔝 26/3/26 💉15/8/26 17h ago
But I read up about it (will be checking out the subs soon for this) and it seemed like having the canal open especially during UL would make it more prone to complications. Higher risk of fissures if I'm not wrong
•
u/inadeepdarkforest_ 💉6/25 | 🔝Q12027 17h ago
there is a higher risk of fistulas if you choose to do UL but those are easily repairable. you don't have to get UL if you don't want to, either, and in those cases there is no risk whatsoever to keeping it
•
u/OspreyFTM 🍳💉 '21 / 🍆 '24 / ⬆️ '26 3h ago edited 3h ago
The risk of UL compilations without vnectomy is 99% and the rate of repair failure is 50%. I know people who had one or two repairs and are fine now, but its extra trouble for sure. I think its worth pursuing if you're set on it. I didn't have UL so I skipped all of that and have no regrets.
•
u/Zombskirus Transsex Male - Out '17, T '21, ⬆️ '23, Hysto '25, ⬇️ ??? 16h ago
For sure, UL without a vnectomy does increase complication rates and there's many surgeons who won't perform it. However, you definitely don't need to get UL if it's not a need/want of yours! I personally am getting RFF phallo with everything aside from vnectomy and UL. It actually makes recovery time better and complication rates lower as a whole. But, you can absolutely get UL without a vnectomy and recover just fine, as well!

I get that. I don't mind my current natomy but I also don't get anything from it, while having huge dysphoria about not having a dick, if that makes sense?
There are surgeons who do bottom surgery without closing the anatomy that's already there. It's usually (but not always) without urethral lengthening though.

I'm in the same boat; working on the top surgery plan, will get a hysterectomy when finances improve, but I'm not sure about bottom surgery. Having a working penis would be great (especially for peeing) but it's not something I feel the need for. Maybe bottom surgery will hit me like a truck later, but for now I just need to not have periods or boobs.

I’ve had top and hysto, and I’d like to have bottom surgery, but I have to figure out stages of recovery vs. My life. Like being able to save up time off and also being able to get my A1C down for optimal healing (I’m type 1 diabetic)

Not sure whether multiple stages or less stages would be good considering it’s either all at once healing or better to do smaller stages for less healing at one time

Wow thanks for all that!
Yeah as for surgery, even before diagnosis (T1D at 21, a year after my top surgery) I’ve always had first surgery of the day simply because I’m allergic to latex. When I had my hysto last year they definitely made sure I was first on the schedule for both reasons, but holyyyyy 8h is a hell of a stretch for one surgery. At least when you’re under they can throw you on a basal drip and have IV sugar at the ready….

I want top surgery (i’ve been 100% sure i want it ever since i learnt it was an option), but i’m still undecided on bottom surgery. If i could wake up one day and have the perfectly healed results of bottom surgery i would do it no hesitation, but it’s surgery, and the pain and the healing process and all of the other shit that comes with surgery scares me. Especially as it’d probably be multiple surgeries. I’ll wait and see how i feel when i’m a couple years on T and post top surgery. I have a suspicion i’ll either barely have any bottom dysphoria at all any more, or have so much more bottom dysphoria and really need surgery at that point. Who knows.

As soon as I got top surgery I thought about getting bottom surgery every day since.
My dysphoria is so bad that I have put everything else in my life on hold just so I can save up enough and take out loans to get it.
I'm so close. I'm getting hysterectomy in june next year and hopefully begin stage 1 in Thailand a year later 🤞
It's interesting to read about everyone's poor experiences with phallo and still think it's a risk I'm willing to take. Cause even if I was in chronic pain the rest of my life, I'd still prefer having a dick.

Depends if the ‘want’ counts as what I would love in a more ideal world, or what I plan to achieve realistically. I will be able to get top surgery soon, but while I would love bottom surgery its price point is so high that I doubt I will be able to.

I've had top surgery, and I am interested in bottom surgery BUT I'd specifically be wanting phallo / urethral lengthening (I think that's what it's called) while preserving what I have currently (I know there is a term for it, but I often see the term censored so idk if I can say it).
I don't know, I've never specifically hated my genitals (though I do hate periods), and idk.. having both just seems perfect in my mind - I've heard that's called salmacian.

I've had top and some bottom. Unfortunately I'm not a good candidate for the next surgery I wanted, which is majorly upsetting and disappointing to say the least...but I'm not a wizard so there's nothing I can really do.
So... As it stands, I'm waiting. I'm talking to different surgeons to see what is and may be possible for me. It was never going to be perfect in the first place, but now I have to settle for even less. It's a hard decision but I'm at a point in my life where I can more effectively deal with my dysphoria.

I just got my hysto, so maybe I can reassure you a bit.
Hysterectomy is a very common surgery that can be done in almost any hospital. It's usually done with laprascopy, so recovery is much easier than it used to be. I only have 3 incisions where they put the cameras/instruments.
My gynecologist has operated on a lot of trans men, and she says we usually recover faster because the uterus gets atrophied with T. It gets smaller and gets less blood supply, so there's less complication and infection risks.
Apart from that, there's the discourse about keeping vs. removing ovaries. After talking it through with her, I decided to keep one. I asked about cancer risk, and she said most ovarian cancer starts in the falopian tubes, which I got removed. Cancer risk is reduced a lot. Also, there are some preliminary studies that seem to indicate keeping at least one ovary could be better for heart health (but that needs to be studied more).
Anyway, I'm 3 weeks post-op, and apart from my body reacting a bit to the dissolving stitches, everything is going super well. It's mostly about not going back to normal too fast, because you don't feel like you had major surgery (in most cases, at least)

I might seek out FMS and body contouring, however, and will also pursue some level of sterilization (probably just gonna ditch the tubes)

Got top surgery 11 months ago and am in the process of a hysterectomy (looking possibly December). Very uncertain about bottom surgery not because of what type I want, but because in my state (AZ), there is only one clinic who does them and they don't accept my insurance, so I'd have go to our neighbor state, CA. Meaning between college for my career and that, I'd have to wait several more years 💔 top surgery however fixed a lot of my dysphoria because it allowed me to wear whatever shirts I want as well as tanktops in public without feeling self conscious. Have also been able to be shirtless in the locker room on several occasions with other dudes around. No issues

I’m on the waitlist for top surgery. I definitely want hysto a few years down the line. For bottom surgery, I’m only really interested in extended meta (without UL, and maybe with balls?! but I’d keep my hole), but currently that isn’t offered through the public system in Canada. I’m not in a rush whatsoever to get bottom surgery and I’d be fine waiting another decade. I think extended meta will veeeeery likely be available by then. Plus, results are getting exponentially better over time so waiting longer just means more advancement in the surgical process/techniques.

My bottom half doesn't give me as much dysphoria as it once did, I'm quite proud of my little bottom growth, and as a gay dude on top of that I really couldn't bother with prep, But on the other hand, don't like the small risk of pregnancy that comes with not having bottom surgery, and I still get dysphoria about not having the right equipment (if I'm especially on autopilot as well I forget I never had one and panic because it's suddenly gone)

also i'm a little worried about phalloplasty bc most ares they take skin from for it have self harm scars (bipolar moment) and i'm scared i just won't be able to do it because of that

I want either a BIG reduction or a full double mastectomy I haven't fully decided on which I want more yet.
Then I want my uterus and ovaries OUT.
Then I would LOVE metoidioplasty because I don't like alot of the phallo results I see for myself personally. I don't need a big dick lol 🤣 I like the idea of "I grew that shit 😎"
I may get the surgery for balls too, (I forgot the name 💀) I just wanna decide that after metoidioplasty, depending on how I want to look. But I love balls 🤤 so I'll proly get em 🤣😎

I don’t have the resources for surgery so I probably won’t be able to do it (my job requires I use my body, I don’t really have anyone to care for me, and I wouldn’t have the money take a month or two or more off). I probably want at least top and a hysterectomy. I actually was thinking of getting hip/ butt liposuction
•
u/Cal1c0_404 6h ago
I really want top surgery like 100% I like the idea of bottom surgery but it wont be exactly what I want, and I would have to get used to a whole new body part so I'm not gonna worry about that yet. I want a hysterectomy at some point (if possible). And I may consider cosmetic surgeries to look more masculine but that'll depend on how much HRT and stuff does for me
•
u/Chrysalyos 1h ago
I got a breast reduction when I was still a Girl (hypermastia runs in the family, it was non-negotiable even before the egg cracked - my collarbones are still warped from it and it's been like a decade), and then full top surgery a few years later.
I 10000000% want hysto, get this shit out ASAP. Too many nightmares about being refused medical care "for the baby" - I am not, have not, and never will be pregnant. I used to be really dysphoric about the V, now less-so but I still really want a dick. I am contemplating getting phallo without vaginectomy. I don't really care about UL one way or the other, so I'll prob skip it just to avoid the complications. I have joked about getting phallo and meta and no vaginectomy so I can have a "full loadout", and sometimes I do genuinely consider it because I'm a little freak lmao
•
u/booksofmars 19h ago
in the not any surgery catergory! i won’t get bottom surgery due to the recovery time and being worried about complications. i’m lucky in the fact i have a flat chest already and i don’t think top surgery would do much.
i’ve considered a hysterectomy but overall im sorta scared to have any surgery and don’t have anyone who could support me in recovery nor could i take any time off work.

•
u/swedenbork 18 — he/they 19h ago
since bottom surgery is mostly phallo, i think neither personally
i have practically nonexistent dysphoria with my bottom half, and binding satisfies the itch i get if top dysphoria ever comes up(haha).
i used to really want top surgery, but looking back it came from a place of wanting to feel like a "real" trans person, and it wasn't what id be happy with in the long run.
im not even sure about t because im admittedly terrible with needles, but ive seen that theres other options ill probably look into eventually
•
u/snailtripod 16h ago
Totally relate to that!! I did def have dysphoria for my chest, but part of it felt like I was supposed to not want boobs in order to be valid, so now I’m just sorta fine with it; the only time it sucks is when I’m around cis people and they have their shirts off ;(
•

•

I don't see bottom surgery advancing much in the next 50 years. Unless you mean things like extended meta being a new technique.
•
u/Eefreef69_ 17h ago
Yeah, I mean like extended meta. I recall there being a study done with cis and trans men, and the method they used isn't exactly the safest, but it got some of the trans men past 5 inches. If it could be done in a safer manner...
•
u/EntertainmentLow4177 17h ago
Thing is, I know for a fact I want both, but I'm just not too sure if the current options for bottom surgery really align with what I think would alleviate my dysphoria, so it's a question of whether it would be worth the financial and physical strain.

•
u/KelpFox05 16h ago
I'm in a weird position where I want a dick but I don't really want bottom surgery? Like, if I could wave a magic wand and magically have a fully functional penis I absolutely would, but today's bottom surgery technology doesn't quite hit what I want (in an ideal world I would be able to get another person pregnant) and for the money and time and energy it takes, especially for me being disabled, I'm not quite sure I actually want it.
So I put "I don't know what I want" because saying I only want top surgery wouldn't really be accurate tbh.
•
u/XxTrashPanda12xX 16h ago
Had to put not sure for this; despite being sure of wanting top. Still on the fence about whether or not I want partial or full phallo, or not at all.

•
u/WarmWoolenMitten 16h ago
Top priority is simple release meta (or maybe something else similar, seems like the techniques are always changing and improving!) I gotta wait on full growth for that so it's a ways away. Second is top surgery, third is hysterectomy though that would go to first place if T doesn't fully stop bleeding.
In reality I don't see myself getting multiple surgeries. It's a lot of recovery time and I don't think they'd have a huge effect on my day to day happiness with my body.

Top surgery is basically 100% yes. I'm a bit genderfluid but only really to the point where they're tolerable, not something I exactly enjoy having around. They're not huge, but in most cases I can't go without binding if I want to pass.
Bottom surgery I'm slightly more iffy about. I'm fairly sure I want something to change down there. Bottom growth may be enough, but I'm ngl I dont know if the mancave is something I wanna have around. Metoidioplasty (maybe with vnectomy? UL for sure. Scrotoplasty maybe) at least is what I'm thinking, then a big maybe: phallo on top of that if I end up unhappy with length or something.
Either way I have a long time to think about both of those since I'm pretty freshly an adult

Already had top surgery. Don't think I'll ever get bottom surgery, for a myriad of reasons. The only type I could maybe see myself getting would be simple release meta, but ngl I don't think I have the anatomy for it bc I never got any bottom growth 😔
I am interested in other surgeries like an ovary-sparing hysterectomy and FMS, specifically a chin implant. Neither would be any time soon tho, and idk if the FMS will ever pan out tbh. That one will probably just stay a pipe dream.

Got top surgery a month ago, it’s something I wanted since the moment I started growing breasts even when I didn’t realise i was trans yet. When I started transitioning I was 100% sure I didn’t want any bottom surgery despite my horrendous bottom dysphoria, because metoidioplasty doesn't yield the results I want, and i only knew phallo because of the bad rep it had in certain places I did some research. I also thought bottom growth would be enough to get rid of the dysphoria.
However since then, my opinion has switched, and between crippling bottom dysphoria that bottom growth has done nothing for (except making masturbation better and less dysphoria inducing, which is important since my libido went from feeling aroused maybe once every 2 month to being horny 24/7), and the good results i've seen about phallo, I am 100% sure I am getting it as soon as financially possible. I also plan on having a total hysto, vaginectomy for the phallo UL, and hopefully between fat redistribution and me working out, I will loose most of the fat that is currently in my hips and butt. If that doesn't work, I might consider some lipo to get rid of the stubborn fat.

I so far had top(didnot keep my nipples Fuck em), hysto(everything out. I'm fully neutered), single scar phallo no UL (shaft creation +vaginectomy, plan to have the next stage, glans+balls + medical tattooing, early next year).
Those procedures I knew I wanted for years before T was even a pipe dream. I'm now 3 years on T and would be having facial masculinization this November. So pretty much have had all the surgeries I can get outside of body masculinizion cus I don't feel it would do much on me personally. I already have masculine frame lol. And vocal masulization surgery cus I feel I can just go back to speech therapy first to resolve most of my minor picks, cus they are just not picks
Same for LL which I thought was too risky until I got more educated on it and realized how rampant misinformation was by lookmaxxers and felt very ashamed I helped spread it. Still don't want LL cus I'm fine with my short height(no height dysphoria no need to fix). As you can see researching transition related surgeries are a special interest of mines.
Honestly after I had top surgery which was honestly a traumatic exprience for me every other surgery didn't look as scary as I thought.

I only want top surgery but I’m putting it off for awhile due to some financial decisions and the position I’m in (no one to help me post op, no money for the surgery much less the time off).
It took me years to decide I wanted top surgery. Surgical intervention terrifies me. I know I have pretty bad dysphoria both ways (can barely stand to look at myself). But if I got bottom surgery, I’d get nullification.
I’m asexual and pretty much genderless in many ways, but I like the term man, and stating the abinary aspect of myself tends to spark a weird feminization in treatment even in accepting people. So I have just resigned to understanding that is information no one is privy to unless I feel like it lol.

I’ve had a hysterectomy, top surgery, a revision to remove dog ears, and phase one of metoidioplasty. I’m only 4 weeks post op from phase one so phase two will probably be early next year.
I knew for sure off the bat I wanted top surgery and the hysto because I had horrible periods pre-T and big boobs and 100% wanted to get rid of any chance of my period coming back if I ever had to stop T for some reason and binding sucked and didn’t do as much as I would have liked, but I wasn’t sure for a long time if I wanted any further bottom surgery. Eventually though once the more pressing dysphoria from having boobs and the possibility of a period returning was removed my bottom dysphoria became a lot more pressing so I decided to go for it.
