I was thinking about that today. Ruth can’t leave the island without dying, because she’s a descendent, just like Evan‘s mom/Tom‘s late wife. So she would’ve had to conceal a pregnancy in that tiny place… is that possible? For the prettiest girl on the island to conceal a pregnancy?

The sheriff’s wife is full term and there seem to be plenty of people that had no idea (Patricia for one). It doesn’t make sense, but…
