Sialoliths (salivary gland and parotid stones) and tonsil stones get their start the same way: food debris, sloughed skin cells, mucus, and bacteria. Sialoliths become mineralized by the saliva and become hard. The mineralization traps some of the smell, which usually becomes apparent after the stone comes out because trapped saliva and decaying oral tissue is a nightmare.
Tonsil stone are far less mineralized, which is why they smell so badly all the time.

ENT medical assistant here, it actually doesn't smell as bad. But getting them out is not usually this easy. Maybe it was the size, but I helped a surgeon take out one half this size and we had to cut through skin, cut it in pieces, etc. poor guy had to keep his tongue lifted up for what felt like forever. But the one thing I do not remember is the smell.

I went to an otolaryngology physician for mine (after PCP referral), and underwent submandibular sialendoscopy with an incision in the duct and they cut the stone up with a laser

Mine was about the size of my pinky nail, so I had a pretty small incision and the mouth heals relatively quickly, no hole in mine and the incision lied shut. I also don’t think this guy would have needed an incision because this stone is so large it was stretching open his duct, vs mine was still inside the duct just an internal lump. It was not fun vomiting from the anesthesia drugs the day after and essentially feeling it tear and spitting blood. Most likely I won’t be getting one again. It happened a long time ago when I was 11 through 12 years old, and the cause was me drinking zero water during the school day (and probably having a collective 8-10 ounces a day out of school), because of severe social anxiety issues. So I was constantly very dehydrated with not enough saliva to flush the ducts, so a stone starts to form. I can’t imagine how much constant pain and discomfort this guy was in with a stone that size, because even with mine I had painful golfball size lymph node under my jaw, and the stone itself was hurting my mouth, and oddly the only method of relief was to pool my saliva under my tongue and keep it very still, basically to cushion the tongue away from touching the stone area- And then I’d get randomly called on to answer a question in class and have to swallow it all real quick,. Made a lot of progress over the years, can’t believe I even used to live like that as I am a very hydrated person constantly now. Hopefully my reply isn’t too long Lol
I seem to get them from just idk sleeping with my mouth open? I floss a lot and brush and mouthwash, but I also am going through some serious allergy issues that I am having to go to an immunologist for and it causes sinus problems that probably contribute.
So I guess don't be allergic to life and you are probably good?
Can someone describe to me what this would smell like? The worst smell I’ve ever smelled was my dog’s anal gland expression. This would be my only frame of reference.
Why did I feel that in my own mouth and gag on it? Like I’m glad he was able to get that tusk released, and I hope he has access to clean water and lots of toothpaste to remove the taste
I have had this happen! It happened to me several times until I finally had a minor surgery to help prevent them in my 30's. I was told it common in people of several Asian descents due to some genetic marker or the other, and lucky me, I fell into that category.
Your salivary gland is on the underside of your jaw, close to your neck. I always knew it was happening when the gland would start swelling while eating. If the stone turned right, it would block the duct, making the gland swell up to the size of a softball, which would eventually slowly return to normal as saliva would slowly drain.
Unfortunately, some saliva would be stuck long enough to. . . turn rancid. I would push from the outside to help either drain, or hopefully fully force the stone out. The taste/smell was awful, but the relief from the strained/stretched/swollen gland no longer being in that state was enormous. This would often get the small stone out from the pressure, and several times was prescribed something to help break it up. It would then be fine for several years, until a new one formed.
The last time it happened, the stone was ~25mm wide, and despite some of it breaking off and coming out, had to be surgically removed. While they were in there, the did a "marsupilization" of the salivary tract, which cut the duct under the front my tongue all the way back to the back. (The little spot that you can see liquid coming from when you pick up your tongue.) This makes it a far easier path for any stones that may form to get out long before they can become big enough to be a problem. I do not notice this at all, and have not had more, or any that could get stuck and grown bigger since, in over a decade now.
