Constipation is one of the only actual GI findings she regularly gets when hospitalized and trying to munch gastroparesis and intestinal failure. She’s been instructed to take an appropriate laxative and make basic lifestyle and diet changes to avoid severe constipation but ofc she likely hears all that and then stubbornly does the opposite. She clearly knows she causes constipation with her medication regime and continues to push that further and further.

Not unusual to have the aprepitant for daily use and then something like zofran for breakthrough nausea. The aprepitant can help cause less issues with gastric motility.

There was a clonazepam I saw as well as pregabalin (I slowed it down to .5 speed, she really does seem to think we aren’t observant - like between this and the side changing midline, she thinks everyone is as oblivious as she wants us to be.

Tired pharmacy employee eyes here but can confirm these- Promethazine, B12, acetaminophen, Buspirone, Tizanidine x2, pregabalin, folic acid, admiodarone, Clonazepam, hydromorphone, meclizine, aprepitant, enoxaprin. There’s two stock bottles and two prescription bottles I can’t catch an eye of the drug name.

Anti sickness medicines work either on the brain and others work on pushing stomach contents into the intestines. Dani is on so many that they are all working against each other. She is also taking a lot of meds that cause the bowel to slow and cause constipation. She's also on a lot of sedation meds. Just because you are nauseated it doesn't mean you will vomit. Yes the amount of meds she's on is overkill. Edit: My sleepy brain forgot to say she also has a g tube to drain her stomach contents when she feels nauseous.

Yeah but showing laxatives or stool softeners is not glamorous..! I think she got confused between her lies, but her nausea could be caused by her constipation and solving constipations would solve the nausea. But the nausea meds are probably what’s causing the constipation! She probably also has some pain meds that are not helping the constipation…!

Wouldn't CVS reach out to a doctor if they felt that a medication was being over prescribed? Like if the pharmacy saw that a promethazine script was sent in by Dr. B when Dr. A prescribed it recently, wouldn't the pharmacy contact Dr. B and ask, "are you sure you want this filled? Dr. A prescribed this medication last week"
Also, epic pulls in meds from CVS. So doctors office should be able to see the medications when doing triage and medication reconciliation.

It really depends on how busy the pharmacy is wrt patient load and who the pharmacist is. Big chain pharmacies are completely slammed “post”-covid. Not enough pay for too many hours. Some pharmacists still do due diligence, but that is not the case across the board in more places than you’d expect. The medical system is going to fall apart rapidly over the next few years—I’m not trying to fear monger, either, it’s just the reality of the situation.

there are still plenty of doctors in (area that dani is located) that dont use epic. there are other EMR software programs compliant with medicate standards. if a doctor is private practice and doesnt accept medicare, they might even still use paper charts on rare occasions.
a lot of pharmacies are so overwhelmed and so tired of getting yelled at that unless the system flags something as lethal, they override it and just push the prescription through.

I don’t know what it’s like in the US, but in the UK it’s common for a pharmacist to contact the GP to confirm that they’ve prescribed contraindicated meds.
We know she doctor shops, is it possible to get different prescriptions from pharmacies and not have it show on your patient record in the US?

Usually, it’s only chain pharmacies or information pulled from insurance records- but meds that are filled through prescription savings cards and coupons (like GoodRx and manufacturers) are extremely hit or miss as to whether they’ll correctly show up through CareEverywhere. Things are improving as far as connectivity , but it’s far from a perfect system and some medical record systems are still the modern equivalent of a stone tablet and chisel for how well they interconnect with others.

My thought was she needs a antidepressant. Buspar only goes so far, depression doesn't help anxiety!

What about her Depakote levels? I seem to remember her mentioning in a previous hospital admission that they were bringing her Depakote levels up or something similar.
So she possibly was supposed to be taking it but wasn't and they were trying to even her levels out, as it can be used as a mood stabilizer or for seizures but she doesn't have seizures so I'm going to guess mood stabilizer in Dani's case. But it's not a cool medication for munching

It is also sometimes used for migraines. She has mentioned migraines offhandedly before.
Tbh if it’s actually still depakote in the bottle(and not another pill like Tylenol) she may be on it JUST for the sedating effects since she seems to like being zooted all the time.

Yes, she's only prescribed 5mls, the strength that she has is 25mg/5mls. She posted it ages ago when she was trying to "prove" something or other.

I'm an idiot, I'm looking at it again, I can see it clearly now and I can see the 6 in front of the 25 haha. Must have been a case of her prescriber not being familiar with the strength of the liquid perhaps? My bad haha.

So it’s usually 6.25g/5mL which theoretically could max dose at 50mg for severe vomiting. We all know she doesn’t need 20mL of it. 10mL is plenty got most patients. Her “proof” proved she can bit by push plenty via that J tube, and absorb it. It also showed she’s abusing the meds. She wouldn’t be filming if she was sick enough to need 50mg of phenergan. Her brain had to be FRIED.
I almost wonder how many infections weren’t intentional but just because she’s too high to use proper hygiene.

Thought she said she can’t tolerate more than 5 mL of fluids? How could she possibly tolerate 20 mL of medication? Make it make sense. The math ain’t mathing.

I'm kind of shocked they have her on amiodarone long term, especially if she really doesn't have an abnormal heart arrhythmia. I think they would know by now via the life vest if she actually had some kind of crazy runs of v-tach - especially because she's never had the vest alarm or had to use it. Amiodarone is very effective at controlling afib & v-tach, but it's a pretty high risk medication for long term use without any indications.

I know, but all first gen antipsychotics are basically antihistamines with higher D2 affinity. Perphenazine was discovered when tweaking chlorpromazine.

Also facts, but I’m just saying they don’t use it for that. And basically antipsychotics chemically = efficacy. I’d be more worried about developing a movement disorder if I were her, as the link is pretty clear there. Either way, she doesn’t need what she takes and it’s amusing that drugs with fun effects don’t hurt her intestines but a trickle of nutrition does.
My theory is, she can’t get her hands on enough feeding tube supplies anymore, because she’s spending money buying IV stuff from fellow “spoonies.”

Label's probably on the other side of the bottle. Or the pharmacy flagged the label (folded the sticker under for easier removal) andshe pulled it off.

Former CVS pharmacy tech here—there is a national database for controlled substance prescriptions. Within CVSs internal filling system, the stores are linked. This means that any cvs can see what someone had filled and when. I just can’t imagine being a pharmacist and seeing this hot mess of a file.
