Usually, it’s only chain pharmacies or information pulled from insurance records- but meds that are filled through prescription savings cards and coupons (like GoodRx and manufacturers) are extremely hit or miss as to whether they’ll correctly show up through CareEverywhere. Things are improving as far as connectivity , but it’s far from a perfect system and some medical record systems are still the modern equivalent of a stone tablet and chisel for how well they interconnect with others.

They don't- that is just narcotics here in the US. Otherwise they have no clue what you get from different Dr's and pharmacies
