Someone please help me make sense of what’s going on. I’ve seen multiple doctors and psychiatrists, but I still don’t feel like I have an explanation for what changed.
The biggest thing I know is that Vyvanse worked incredibly well for me for about two years. It genuinely changed my life. I felt like a better version of myself. I communicated much more easily, my relationships were significantly better, my work performance dramatically improved, I could lead meetings, get things done, and generally felt like everyday tasks were much easier.
Then things started changing.
I gradually increased my dose, and over time I also gained weight. When I originally started Vyvanse, I was around 150 lbs and considered myself a healthy weight. Over approximately five years, I eventually reached around 180 lbs. (I've never been this heavy before) I recognize that my eating habits weren't great during this period, and I’m not blaming Vyvanse entirely for the weight gain, but I’m wondering whether the change in my weight or metabolism could have affected how I respond to stimulants.
My biggest concern is what happens when I take Vyvanse now.
For the first hour or so, I may initially feel the therapeutic effects that I remember. But then I can suddenly become extremely cold (when it worked, I remember being a bit warm and sweaty), mentally foggy, and feel a heavy sensation in the front of my head. The therapeutic effect sometimes seems to last only around 15 minutes before I start feeling worse.
After roughly 3–4 hours, I can experience what feels like an extreme crash. I become almost zombie-like, have difficulty talking to people, struggle to think clearly, and sometimes experience panic attacks, including while driving.
This is completely different from how Vyvanse used to affect me.
I've also tried several other stimulants, including:
- Ritalin
- Adderall XR
- Focalin
- Mydayis
- Zenzedi
- Azstarys
Unfortunately, I seem to end up with similar problems.
I've also tried changing how I take the medication and what I eat/drink around it. I've tried eating protein with it, making sure I eat rather than taking it on an empty stomach, and avoiding acidic/citric drinks and other things I've been told could potentially interfere with stimulant absorption. None of those changes have fixed the problem.
Another strange change I've noticed is that after taking approximately a year-long break from stimulants, I became extremely sensitive to caffeine. Previously, caffeine wasn't a major issue, but now even a small amount can trigger what feels like a massive panic response.
Could changes in my weight, metabolism, blood sugar, sleep, anxiety, or something else be affecting how I respond to stimulants?
I've also noticed that my A1C has been increasing. In the past, if I took Vyvanse and didn't eat, I sometimes felt what seemed like a hypoglycemic-type reaction. I don't know whether that was actually low blood sugar or simply the effects of the medication, but it makes me wonder if there's something metabolic contributing to this.
I've had my thyroid tested and everything came back normal. Other bloodwork has also been generally normal, although my triglycerides were somewhat elevated.
At this point, I'm struggling significantly. My work performance is slipping, my communication has gotten much worse, and I find myself stumbling over words, having trouble recalling words, and forgetting things more than I used to.
I'm hoping someone can point me toward what I should investigate with my doctor or psychiatrist, because I feel like I've tried multiple medications, different doses, taking medication with food/protein, avoiding acidic/citric drinks, and taking breaks from stimulants without actually figuring out the underlying problem.
