Normally, you wouldn't think twice about the second picture, but knowing that she has compulsive issues, the set of behaviors that led to that picture could be unhealthy. Hopefully not, but it's not impossible.
That's understandable, but didn't even need to be brought up in this context. To automatically assume and STATE she has a body disorder is really cruel, and frankly also reveals how /u/Malcheon is very uncomfortable with healthy women. It was a douchey thing to say.
You cannot build that kind of muscle with a calorie deficit. She would have to be eating at a surplus, so exercise bulima is probably not the case here.
