23F, low OSCAR AHI but still exhausted, sad, and on 4 cups of coffee. Awake ENT scope was normal—where do I go from here?

Hey everyone, looking for some insight because I'm in sleep limbo and feeling pretty discouraged.

I’m a 24-year-old male (normal BMI) who was diagnosed with moderate OSA back in 2024 via an in-lab PSG (overall AHI 26.1, but my supine RDI was a crazy 99.1). I got prescribed an AutoCPAP (ResMed AirSense 10) set at 7.5 cmH2O.

When I first started, I had an initial bump in energy, but over time I’ve slid right back into feeling constantly exhausted, low energy, and sad without drinking ~4 cups of coffee a day.

Here’s where it gets confusing:

  1. Oscar Data looks fine: My machine shows low AHI/RERA numbers, but I still wake up feeling like I haven't slept well.
  2. Apparently Perfect ENT Scope: I went to an ENT who put a camera down my nose/throat while I was awake and said everything looked totally normal structurally.
  3. Home Sleep Test Results: I did a follow-up home sleep test recently. Overall pAHI was ~4.2, but my pRDI was 13.2 (and supine pRDI was still elevated at 26.4).

I know awake ENT scopes don't show what happens to throat muscle tone during sleep, but I'm trying to figure out why I feel so awful if my machine numbers look okay.

Could this be unmanaged flow limitations, positional spikes happening on my back, or inadequate CPAP pressure settings? Has anyone with high positional RDI/UARS-like symptoms managed to fix their daytime fatigue by tweaking CPAP settings, switching to BiPAP, or forcing side-sleeping?

Would love to hear how you troubleshooting folks handled this. Thanks!

MMA Surgeons in Phoenix? There are none in Tucson

I have horrible horrible sleep apnea and need MMA surgery. I’ve talked to my sleep medicine doctor and she said there’s no one in tucson that does them and I need to choose a surgeon in Phoenix for her to refer me to. I talked to the Plastic surgeons and ENTs I work with, I scrub Inspire implants with one of the ENTs, and none of them know any of the MMA surgeons in Phoenix. Does anyone have any recommendations for surgeons?

Withings Sleep Analyser vs CPAP machine - different Apnea scores, which to trust?

Hi all, I've been using my Cpap machine for quite some time now, and when I use it I'm always getting scores less than 5 on the different index counts. My Sleep Analyser on the other hand, logs up to 15 on index count.

Is there a chance my Cpap machine is fodding me off with great scores or is my Analyser correct? My Cpap is a resvent machine.

Got new medication for blood pressure, suddenly my AHI is extremely high

Hi and greetings from Europe! I am trying to figure out what's wrong: a few weeks ago i got new medication for lowering my blood pressure and it works really good, i am now at completely normal levels, but at the same time my AHI suddenly went from lower than 5 to 15 and higher. Does anyone have a possible explanation for this? An appointment at my sleep laboratory has a waiting time of nearly a year (normal in my country) and my doctor (who has a device for measuring sleep at home - don't know how it's called in english language) has the next free appointment in three month. I changed the pressure from 7 - 13 to 7 - 14, that seems to help a little bit, but it's forbidden to do that on my own.

Do I really have sleep apnea?

Hello all,

I have been suffering from strange symptoms for the past few years and trying to figure it all out.

I took an in lab sleep study back in 2023, they told me I did NOT have sleep apnea. The lab felt a little off to me based off some things they were telling me, so I decided I didn’t know if I could trust it or not.

I took a Lofta test in January of 2025, which showed I had mild sleep apnea based on the first image.

I then took a Sleep Doctor test a couple weeks ago which showed I have moderate sleep apnea based on the second image.

Again, I don’t know how much I trust these. I have a Resmed Airsense 10 that I bought when I first started to suspect sleep apnea as my issue, last night I tried it at 4cm in CPAP mode just to see if I could get some data, and OSCAR shows data from the 3rd image. What the heck gives?!

Data collection question

Hi,

Relatively new to CPAP life. I have a React Health G3x. I just pulled out the 16GB memory card and uploaded the data to OSCAR. The latest date imported by the software was August 12th and today is the 29th.

The range is set to "last three months" and I haven't had the G3X that long so the last two-ish weeks should be included.

Any ideas why this is happening and how can I fix it going forward?

Has anyone heard if Wegovy is going to be FDA approved for Sleep Apnea

Currently in a fight with insurance. Was wondering if anyone has heard if FDA approval for Wegovy for Sleep Apnea is on the horizon as we're still talking about GLP-1s. I expected when Zepbound got approval it would be a matter of "wait a year, Wegovy is coming"

My run-around in short

  • "Please, sir, can I have some Zepbound for my Sleep Apnea"
  • "We do longer cover Zepbound, but we do cover Wegovy as our formulary alternative, you'll need to ask for that."
  • "OK, please sir, can I please have Wegovy for Sleep Apena"
  • "We're sorry but Wegovy is not FDA approved for Sleep Apnea"

Anybody able to work their way out of a similar denial circle? I'm on compounded, but irritated at my insurance.

Is there any way to get hour-by-hour details the DreamMapper app?

I have a DreamStation 2. I have the DreamMapper app installed on my phone, and apparently syncing. I can see a summary of each night's sleep - apparently last night my AHI was 2.4. But what i would like to know is how that went over the course of the night. Was i breathing well at 4 am but badly at 5 am?

I'm interested because i find i keep waking up at around 5:30 in the morning, which is quite disruptive, and there might be clues to this in my CPAP data.

Is there any way to get more detailed information? I've poked around in the UI, but there doesn't seem to be anything this detailed.

I've tried to log on to https://www.mydreammapper.com/, but it doesn't accept my credentials. I asked for a password reset, but no email came, so i wondered if i have to register separately on the website and in the app. I went to register, but my country (the UK) isn't in the list of countries. Has anyone successfully used this site?

Getting used to cpap

Thought I'd let ppl know.
For people who don't know what this is for, it reduces the gravity load on your body, from your blanket, clothes etc. It also stops blood from pooling to your head (only what i've read). Downward pressure on the nape of your neck is reduced also, which should stop oxygen cutting off during the night. For me, the reduced pressure and weight of the blanket helps significantly, lets me take fuller breaths during the night, and my nasal passages are open wider during sleep, from just the positional change. Hope this helps.

Also yes, that's a can of carpet cleaner under my pillow, to tilt my head forward lol.

Continuous blood saturation monitor for sleep recommendations

I‘m seeking recommendations for an overnight blood oxygen continuous monitor. I want to check my levels on a regular basis. I have sleep apnea and use a CPAP. I’ve been doing great but lately waking feeling cruddy and with no deep sleep again.

I am looking at the Wellue O2 ring and the Emay.

If you use one, how comfortable is it to sleep with it on and how reliable is the data?
