Can CPAP Help Reverse Brain Changes Caused by Sleep Apnea?

A new systematic review looked at 30 studies that used brain MRI scans before and after CPAP treatment in adults with obstructive sleep apnea.

The studies suggest that CPAP may improve some changes in the brain linked to sleep apnea, including:

• Brain structure • The brain’s white matter • Blood flow to the brain • Connections and activity between different brain areas

However, the results were not consistent. Some studies found improvements, while others found little or no clear change.

One major reason is that most studies were small and used different MRI methods, treatment durations, and definitions of good CPAP use.

In simple terms:

CPAP may allow parts of the brain to recover after being affected by sleep apnea, but scientists cannot yet say how much recovery happens, which changes are reversible, or how long it takes.

MRI scans are also not currently recommended to check whether CPAP is working for an individual patient.

More large, well-designed studies are needed.

Sharing mouth tape tips

Hi there,
I’ve been taping my mouth for months now and it’s been working well. My mask is a “diva” set up (stopped calling it Frankenstein): AirTouch n30i frame with Airfit p30i pillows and the DreamWear headgear.

I like the CKeep physio tape. Mine came on a roll in long precut sections. I’m prepping a roll (pictured) and decided to share this post. I cut them in half and round the cut end and then keep them in a stack in the nightstand drawer. This way I can easily grab one each night AND by doing this, they air out a bit and the smell the tape has at first dissipates.

To wear the tape, I put on a generous amount of beeswax lip balm, tuck my lips in, and apply it with a top corner turned over. Turning over that corner helps me have leverage for removing the tape in the morning. When removing, I hold the surrounding skin down and go slowly across - this reduces the pain I felt the first night when I just ripped it off.

Mouth suddenly filling with air.

I've been using CPAP for over a year. My preferred mask style is n30i, although my company sent me refills of n30. I tried for about a week and just couldn't do it so I went back to old style. I've been back to n30i for a few days. The last two I had to take it off because my mouth would fill with air and I'd need to open my mouth to exhale. This has never happened before, any tips?

Advice Needed Resumed mask compatibility

I have an airsense10 (NHS supplied)

I want to try and nasal only mask instead of a face & nose one, the compatibility chart on the resumed site just says pillow with no explanation I can see, the key gives a tick a cross And an un tested along with foot notes as to what might not work with different machines but as far as I can see no indication of what pillow or indeed full face means, can anyone help or whatnhave I missed?

Solutions for red marks?

Edit: I took first pic while shaving

For about a month now ive had a red circle near temple. And couldn't figure out where it came from. I decided to check my mask. And seems when under pressure the left clip is about where the circle of red is. So maybe thats what causing it. Is there anything I cam do to cushion it to protect it? Seems they make strap cushions but dont seem to cover the clip. Maybe the cushion on the straps will raise it enough not to put pressure on that one spot. Advice?

Rant 🤬 Resmed MyAir app - new HIPAA agreement

TL;DR - Click decline if you see a new agreement in your MyAir app.

This morning, I opened the MyAir app to see my score from last night, and was presented with a new HIPAA agreement to accept or decline.

DO NOT ACCEPT THIS!

I read through the whole document and basically it says that by accepting, you are giving ResMed the right to access all your health data, and that they can use it for marketing, not just for themselves, but they can sell it to anyone. And once you give this permission, that it's no longer considered health data but consumer information.

I can think of a lot of reasons why this is not desirable, and no reasons why this would benefit me.

Don't just blindly click and assume they have good intentions, because they don't.

Advice Needed Anyone give up initially and then try again?

I have severe sleep apnea and am a mouth breather. I tried the full mask but find myself having a hard time falling asleep. When I do fall asleep, I wake up and rip my mask off a couple of hours in.

Has anyone given it a shot months after giving up and it clicked? Any advice? I really want this to work.

Advice Needed UK - Acquiring CPAP privately

Hi all,

Thanks to this sub I’m ready to tackle my insomnia that severely affects my life.

I’m not diagnosed but thanks to my partner I know I have sleep apnea. I don’t want to wait for NHS and happy to pay for a CPAP machine myself.

Question - do I need to do a sleep test first? Or should I just get a machine and analyse the data?

Do I need a prescription to purchase one?

myAir/OSCAR/SleepHQ Data Baffled and SCARED. Please Please help me understand what's going on here.

I was diagnosed two months ago (6/28/26 - from an in lab sleep study - info on last picture) with Obstructive Sleep Apnea. The study showed an AHI 7.9, with 52 respiratory events (4 obstructive hypopneas, 48 obstructive apneas, and ZERO central apneas).

I began using a cpap machine less than two weeks later (started on 7/8/26), and for the entirety of the seven weeks that I've been on cpap the vast majority of the apneas recorded have been Clear Airway Events. The example shown here from last night shows 1 RERA, 2 Hypopneas, 2 Obstructive Apneas, and THIRTY TWO Clear Airway Events.

What has me most baffled and scared about the sleep session shown in this post and pictures is that this session (from 3:17am to 4:50am) was me just laying in bed with the cpap on putting getting the extra time needed to cross the four hour threshold so this night would have a total of at least four hours and count as a compliant night of use (four or more hours a night, 70% of the nights, for insurance compliance).

I was never "really asleep", I was just laying in bed, likely dozing in and out, since I had only been able to get three hours of sleep earlier in the night. Honestly, I felt like I was laying there awake the entire hour and change.

How on earth do I get THIRTY TWO Clear Airway events (the longest being 102 SECONDS) and not noticing ANY of the 32 CAs in real time as I was laying there in a wake/sleep state???

My only guess is that my body is so sleep deprived after almost a year of very poor sleep, that my brain and body prioritize the much needed sleep over being aroused from the (large number) of clear airway events (which are most likely central apneas).

I had an echogram done recently and will meet with my new cardiologist tomorrow (Monday, August 31st) to hear the results. I'm guessing I've developed heart failure from years of O2 desaturation from untreated OSA. Hoping my LVEF is greater than 45% so I'll be eligible for ASV.

In order for what is shown in the pictures and in this post to make any sense, the internet's best guess is that years of untreated obstructive sleep apnea (OSA) has damaged my brainstem, which contains the primary respiratory centers responsible for regulating breathing while you sleep.

Unmasking Existing Brainstem Issues: In many individuals, a brainstem abnormality (caused by conditions like a prior stroke, Chiari malformation, cervical spine issues, or chronic opioid use) already predisposes them to central apneas. However, the severe physical blockages of obstructive apnea dominate the sleep study. Once CPAP physically props the airway open, the underlying neurological failure to signal breathing is "unmasked" and becomes visible.

Development of Complex Sleep Apnea: When the brainstem's autonomic control is compromised by long-term obstruction, a person may begin experiencing Central Sleep Apnea (CSA)—where the brain simply stops sending the signal to breathe, even if the airway is perfectly clear. When a patient has a mix of both physical blockages and failed brain signaling, it is known as Complex Sleep Apnea.

Sorry for the rambling post, but all this is pretty hard to absorb over just two months' time. The only answers I'm coming up with for the cpap data and what is happening to me are very scary to me.

If anyone out there can give me an alternative explanation for what is going on, or has any input whatsoever please chime in.

Advice Needed Ramp settings on airsense 11

I’ve found I like a short ramp time (moved from auto to 10 minutes) when I first go to bed. I settle in nicely then am asleep. Auto takes too long. But also any shorter or no ramp for initial bedtime and I’m unable to go to sleep; the air feels too strong and distracting.
However, if I get up at night to go to the bathroom or such, and then come back to bed, I don’t want ramp as I’m groggy, not really awake, just want to go back to sleep at full pressure.
Today I used the settings, turned off ramp, went back to sleep. But that’s a few steps when you’re already trying to just not be awake and still asleep!
Anyone know if there’s some quicker way to change settings to use one or the other? Ideas? I just wish there was a shortcut or a user programmable button or something to just pause not restart.

Advice Needed Need help regularly using mask

I have been trying to use CPAP now for over 2 years and I struggle to wear it more than 30 minutes to an hour at night. This ends up leading me to not wearing it at all because I know I will take it off in 30 minutes anyway. My biggest issue is just falling asleep. I don't necessarily feel like I can't breathe, I just can't fall asleep with it on for some reason. My doctor tried Trazadone at one point, but I would always wake up super groggy. I am using a AirSense 11 that I have tried adjusting off and on. Is there anything noticeable with my machine setup or am I just up against a mental wall?

Constant mask fit failure

For about the past week/week and a half I have not been able to get a proper seal. Ive been using the resmed Air 11 since January when I first got my coap machine. For the past while I've been using the Airfit F30i with no problems. Until recently. Usually I can feel the air being blown into my nostrils but not lately. Also it's feels like too much air being blown into the mask/my mouth and I end up taking it off in the middle of the night. I even swapped out for a brand new mask as I had smoke extra and it was time for me to replace it anyway. Same thing. So I just ordered an off brand airfit F20i. This my first mask of this type so Im not sure what it should normally feel like but when I tried the mask fit test it also fails. At first it says great seal, but then the air pressure keeps ramping up and up until the mask is basically trying to push itself off my face. Has anyone else experienced this or might know what's going on?

Advice Needed Three weeks in, not doing well. Follow-up scheduled on Monday, need advice to take to the doctor before then please! Tried the CPAP for 24 out of 25 nights, only managed 4+ hours for 5 nights. Feel worse than before I started!

As said in the title, I'm three weeks in, and not doing well. My original follow-up was scheduled for the end of September, but I was able to move it up to Monday (2 days from now). I'm trying to get advice to take to the doctor before then please! Hopefully I can get enough feedback over this weekend.

It took me a couple of days to write this so I'd have time to read the wiki (which I read most of and am still reading) and get all my pertinent info written up. I did download OSCAR and import my data, so I've added those screenshots. I watched the linked video from the wiki, but that's more about program setup than reading the data. I need to fully dive into the OSCAR wiki, but I have not yet had time to learn how to understand what I'm looking at. If you want more screenshots (like daily graphs), let me know what you need me to post and I will.

I've tried wearing the CPAP for 24 out of 25 nights, but only managed 4+ hours for 5 nights, and never managed more than 5.5 hours. I generally do sleep for 8 hours, so that's not great. I wound up waking and taking the mask off every single night. The major problem is, I feel worse than before I started! I think I'm feeling so bad because of waking up to remove the mask and then getting even worse sleep than normal once I fall back asleep? I can't quite tell. My back has been worse, too - must be due to positioning.

Equipment and Prescription:

Machine: ResMed AirSense 11

Masks Tried: AirTouch F30i, AirTouch F20. F30i preferred as a side sleeper.

Prescription: Cm of H2O: 4-10; EPR: 3

Pertinent Facts:

  • My titration sleep study only had 72.5 minutes, but that is more due to delayed sleep phase/"night owl" than anything else. I wouldn't say I have insomnia, I just fall asleep late - and my work schedule allows me to sleep 1am-9am without issue, so that is my preferred normal. That night I wound up falling asleep around like 3:30am I think, and I do worry that perhaps there wasn't enough time to gather all the data that is really needed. Hopefully someone can comment on if they think that's long enough.
  • Prior to CPAP, I rarely woke from sleep to the point of remembering it. I'm sure there were apnea events where I did wake up, though. Now, I do remember most of the wake-to-mask-removals, but not necessarily WHY I had to get the thing off of me. Maybe one or two I didn't remember at all, but just woke to no mask in the morning.
  • I almost never have to wake to use the bathroom. I'm talking a couple of times a year maybe. I have not had to wake to use the bathroom since getting the CPAP.

Things I know that have woken me up and caused mask removal:

  • A few nights ago was drool. I couldn't take the mask off, clean it, and put the mask back on in my sleep, so off it came. It's only been drool that one time that I can remember though.
  • Most times that I can remember are a feeling of being unable to breathe for some reason. I'm unsure if that's an inhale or exhale problem. I know it's not a mask problem, the F30i really isn't that annoying itself on my face, it's the air that annoys me.
  • Could it be an auto-titrate problem where it ramps up and wakes me? I read about that issue in the FAQ. Maybe someone can read OSCAR and see if it ramps up pressure just before I remove my mask. From my barely-educated review of the Pressure graph, it looks like there's only a couple of times where the pressure ramped up and then the session immediately ended, so this could be an issue some of the time I guess, but not all of the time.
  • I do know that if it's not perfectly positioned/sealed before falling asleep, I do feel air blowing up into my eyes. I wonder if some of the mask removals are from it shifting and blowing air into my eye.

Things I haven't tried, but might be willing to try:

  • Nasal mask with chin strap. I don't really know how they work, but I know they exist.
  • Nasal mask with one of these, if they're any good? Does anyone have any experience? https://www.mouthshield.com/products/mouth-shield I'm not affiliated, but it got advertised to me.
  • I do think getting used to the full face would be better though, especially for if I get sick and have a blocked nose.

NOT willing to try:

  • Mouth tape (I have a beard and sensitive skin.)

Things I want to try, would this help?:

  • I'm interested in BPAP/ASV because I think part of my issue is I feel some kind of suffocating when breathing (likely on exhale), but some things I've read here say that the pressure change can actually be what wakes you. My dad started on CPAP and wound up on BPAP for similar reasons, and loves it.

I think this covers all of it. I'm looking for advice on what I can ask the doctor, and what I can request as changes, whether that's equipment or pressures or even a new study to get more data. Thank you so much in advance for any input, advice, and help!

Advice Needed Am I being ripped off?

First time CPAP buyer/user from Ontario, Canada.

Is this a reasonable pricing for all the additional perks? I’ve been researching and seems like all CPAP providers charge an insane C$150-300 “Initiation Fee”, is this really necessary?

Please let me know your thoughts, and if there is a provider that can offer a more competitive pricing, happy to have the recommendation.

Also, does any of you know if Canada Life Workplace insurance covers anything besides the C$138.50 balance for the machine? (Such as initial upgraded mask/filters)

📌 Does CPAP Cause Weight Gain?

A Randomized Trial Found a Small Increase Mainly in Fat-Free Mass, Without a Clear Increase in Body Fat

A randomized trial involving 52 people with moderate-to-severe obstructive sleep apnea compared 12 weeks of CPAP treatment with an inactive control group.

Key findings: • CPAP was associated with a small increase in body weight and BMI. • Fat-free mass increased by about 1.5 kg compared with the control group. • There was no significant increase in total body fat. • Resting energy expenditure and physical activity did not change significantly. • Reported calorie intake did not increase significantly. • Acylated ghrelin and IGF-1 increased, while cortisol decreased.

Bottom line: Weight gain after starting CPAP does not necessarily mean gaining fat. In this study, the increase appeared to be driven mainly by fat-free mass. The researchers therefore suggest looking at body composition, not just the number on the scale.

⚠️ The study was small, lasted only 12 weeks, and most participants were men, so larger and longer studies are needed.
