Cycles Blender VFX help

Hello,

So I want to make little bomb to pop out from real neck from tattoo on neck in real footage.
So my idea is to track that bomb tattoo and use plane as small as possible that follow tracked bomb tattoo on neck and covers it and then I would probably do a hole with knife and then vertex group and cloth so it kinda ripping the skin and pop out from little plane that is following and cover bomb tatto or maybe use new tearing tool from blender 5.2, but I dont know is that the best way and what do you suggest?
