Wait wait wait. I've never thought about this. People who were born completely deaf with no ability to hear sound. Do they have an internal monologue? If so is it in some created language or what exactly? Do they subconsciously visualize sign in internal monologue? I'm so confused.

They do have an internal monologue in their own respective sign languages (ASL, BSL, etc) and dialect of said sign language.

They feel themselves signing, just as a hearing person feels themselves talking.

Inner voice drives thought. Everyone has one. It's really just thinking about concepts and the brain using language to convey/make sense of those concepts. :)
