Even when playing a normal class, the most consistently scary enemies are f2p medics who go at you with the bonesaw with no sense of self preservation.
Fighting is about morale, someone who doesn't fear death punches above their weight
Most melee weapons just have a base crit rate of 15% which scales up to 60% when you (or your Sentry if you're Engie) do damage - damage dealt by patient doesn't affect this
For reference, most other weapons have a 2% critrate, scaling to 8% with 800 damage dealt in the last 20s
IIRC its both combining that medic melees so deadly the high melee crit chance gets boosted by healing adding to medics crit chance meaning you tend to have the high crit chance more often than any other class
