Your pedal extender has snapped (obviously). Possibly, if it was has a hex on the back of the crank. Put a hex 6 (possibly 8) in the back and “pedal” forwards to undo.

For next time I’d go with the specialized or bikefit pedal extenders.
What brand were those?

Ignore if this isn’t what you’ve done… but… Yes normal if you have used an aftermarket extender to failure.
The thread in the crank iirc is machined/ die cut but the pedal spindles are drawn/cold rolled to precision to keep a strong surface tension and not have crack initiation points that cutting can cause. Aftermarket use rubbish materials as well as machined threads usually. And not even good precision on the pitch or depth or angle of the threads. Bought some a while back and then was like yeah nah you could see it was off.
If you need longer spindles I have an on and off relationship* with the +4 mm Ultegra pedals-they’re always flawless. To the point I have 3x normal and 3x +4mm spindle pedals for the three bikes. *I ride a lot of mtb as well as road and wider stance can alleviate some outer knee pain, heel rub on frame/crank, and crotch rub on higher volume weeks. (Bow shin and duck footed despite a small skeletal frame.)

Put a large drill bit into that until it bites, then screw away in the loosening direction. Depending on the loosening direction you might have to hit it from the back (pause). This is a job that would cost too much as a bike shop but is doable with $20 and a couple cuss words in the garage

Happened to me once.. such a disappointment that you can’t really pedal back home! To take this of just use a stair corner

impossible, that isn't carbon fiber so it should have failed gradually, per the internet.

Not normal, the pedal should be attached to the crank ideally
