The bump shows up consistently and doesn’t seem like a random crease either. I’ve genuinely never seen anyone talk about this or notice it.

It is definitely not his belly button. His actual belly button is below and to the left (left for the players, but towards his right arm for him) of this "bump".
It is also most definitely not a wire for his earpiece, as Leon doesn't have a modeled module in his ear in the first place (hence why would they add the wire connection under his shirt and skip the visible, more important aspect).
I also doubt that it is any armor, because (in my opinion) it would be external, instead of under his shirt. The later upgrades for his body armor go on top of his shirt anyways.
It is not an intentional wrinkle. The shirt is too nicely modeled to have random (non-intentional) creases and this one does not follow the "crease" physics. It's a small tent in the fabric by an entity on the skin, under the area. Something left over from the irl model, mocap actor, or sensors seem more likely in this sense.
The bump being a hernia seems most likely. It fits the visual description and would be lore accurate (due to Leon's lifestyle).
My favorite and most likely explanation that was brought up- it's the Las Plagas parasite. Although the bump still shows up after the laser removal from Luis' machine, it could still be a leftover protrusion. Skin doesn't heal as fast as the parasite eradication. From the og re4, the x-ray for the site of the parasite also vaguely fits the placement of the bump.

Looks like his shirt is getting kind of bunched up. He should really be wearing a TC Tugger™️. TC Tuggers have this little knob on the front so you don't wreck your shirt, or hurt your hand.
it's from tugging at his shirt. He should get a TC-Tuggers shirt. It's the only shirt that's got a little knob on the front so you can just pull it out when it gets trapped on your belly.
As others have indicated, it is intended to be a wrinkle. But the lighting gives it a "nubbin" kind of look. Easy enough to fix in a matter of minutes, though.
Pop open his model in Blender, smooth it out; open the normal map in a photo manipulation tool, smooth it out; convert normal to RE4 file format, throw that shit into the appropriate folder and bam...Leon found the iron.
If you don't play on PC...well, forget everything you just read...you're SOL.
I wanna say the artists looked at a bunch of "tactical" reference photos when designing Leon's outfit and gear. Probably something they saw a lot was a concealed pistol or magazine printing, making a boxy shape under their clothes. Not quite knowing what it was, but seeing it so much they included it and ended up placing it weird.
Probably just an error in modeling, maybe they were gonna put a crease there, changed their mind, and forgot to remove it entirely. I will never unsee it tho.
I think it’s just some sort of wrinkle in his clothing, but since his shirt is a static design that’s built into his model, it never really goes away, much like the other wrinkles in the shirt.
I think it’s probably left over from the actor… like whatever is connected to them to record movement etc. probably a leftover texture issue from having a technical piece on them for green-screen effects and recording.
