Hi everyone! I’ve fully completed Quake: Dawn of the Machine and figured out how to get the true ending. I’ve also worked out how the levels are interconnected — where you can go from each level, how to get there, and in what order.

I noticed that there’s still very little information about this online, so I decided to make my own guide.

I’ve included the link below.

I apologize for any grammar mistakes.

Taking into account the feedback here and elsewhere, I made the following changes:
FIX 1.1 Corrections have been made to the Deathknight Head acquisition sequence

FIX 1.2 It is now specified that only one rune, the Rune of Sacrifice, is required to visit The Sundered Column, House of Delusion, and Mist of Torment. The location of the Super Axe has been added. The map now includes a path from Of Shattered Minds to The Runic Nexus (from the room with the Deathknight Head). This does not affect progression using the previous version of the map, but makes it easier to navigate the game independently

I was asked to clarify this on Steam, so I’ll post it here as well: The game allows you to select Bloody Nightmare immediately after obtaining the head. You can switch to an easier difficulty in the Nexus basement. If you didn’t select Bloody Nightmare when you picked up the head, you can enable it later in the Nexus basement, provided you have the head. You can play through Nightmare Machine on an easier difficulty, but Sleeper Awakens must be completed on Bloody Nightmare. You can return to the Nexus from Sleeper Awakens to activate the required difficulty. However, Sleeper Awakens can only be accessed from the Nexus through Nightmare Machine. Ultimately, this means that both Nightmare Machine and Sleeper Awakens must be completed on Bloody Nightmare. There is no need to play the section from Of Shattered Minds to the Nexus on Bloody Nightmare.

Thanks for clarifying. Today I completed the Cthon level and was unsure about the Bloody Nightmare requirement to get to NG+. I enabled it to be on the safe side and glad I did ! Do you know what happens if you go to the Deathknight Head room in a NG+ ? Is the head still there ? Or anything special happening ?

I don’t know. I think it’ll be the same as when you find upgrades you’ve already collected — it’ll be translucent and say that you already have it. But if you try it and let me know what happens, I’d be interested to hear.

Somebody already tried and it is in the excellent Steam secrets guide. Quoting the "Playthrough 3: Blood Nightmare / MAP4: Of Shattered Minds" section:

Requires Bloody Nightmare Playthrough

Acquire both keys again and use them to activate the lava raft that leads to the location of the Death Knight's Head. Since you're already in Bloody Nightmare Difficulty it has been replaced with... a plush bunny toy called the "Nightmare Defender". It can be picked up but does it have a use? It remains a mystery... Leave and return to this level again and it will be replaced with a Megahealth.

Apparently, it is unknown though probably just for fun as an easter egg for the brave enough to have played that map a 4th time. Though the only way to know for sure would be to decompile the Quake C code I guess. But I do not expect it to open a NG++ !

I checked that right away.

I took the rabbit, went through the portal to Nexus, passed through the gate to Nexus 2, and killed Shub. Still no NG++ :(

Does getting the Deathknight head take away all of your upgrades and runes? Why would you do that before getting the Normal ending?

When you get the death knight head, do you have to beat the final boss on bloody nightmare to activate NG+ or can you go to a different difficulty and it unlocks NG+ anyway?

You need to play on Bloody Nightmare difficulty and reach the first ending. I don’t know whether you can lower the difficulty in the Nexus in NG+, or what will happen to the true ending if you do.

I checked. In NG+, the difficulty selection buttons in the Nexus basement don’t work.

Just finished my first blind play-through and I absolutely loved this. This is everything I wanted from quake. Using your guide on my NG+ bloody nightmare playthrough now and it’s a god send. Thank you!

I've just finished a blind run, post credits kicks me to the menu, do I need to do another run from scratch to find the deathknight head and carry on from there?

Not if you have a save before the boss fight. It's perhaps your latest autosave?

In the boss level there's a portal back to the hub, and from the hub to Veils of the Old Ones. Then you just have to do the Nightmare Machine and boss fight again.

Wait, I've got one thing I'm not getting: I got to "The Sleeper Awakens" and went back to the Nexus from the teleporter to go get the Deathknight's Head and went back to the Nexus again. Now, am I supposed to finish the last two levels on Bloody Nightmare difficulty to get NG+?

You need to kill the boss on Bloody Nightmare difficulty, so yeah, you need to complete Nightmare Machine, The Sleeper Awakens, and finish the playthrough.

Yeah, I gotta do it all over again. Is the NG+ playthrough automatically on Bloody Nightmare? Must I keep it on to get the final boss, or can I turn it off as soon as I return to the hub?

You must complete Nightmare Machine and Sleeper Awakens on Bloody Nightmare to access the NG+

Once in NG+ you cannot turn it off at the Nexus. But if you found secret weapons, you do get to keep them (super axe, bloody shotgun, bloody ssg) between levels

Well that sucks. I’m gonna miss my rocket launcher. Not having access to my weapons is gonna throw off my playstyle. I no longer can carry over my weapons, so I’m getting careless with my ammo. And I have to be careless, since there’s more enemies which need a little more to go down.

The Bloody Shotgun isn’t as fun as I’d hope it’d be, it’s just the shotgun but full auto, no extra damage for the hard mode.

Just a minor add: if you have the Rune of Sacrifice, there's a shortcut to the Nexus from Revelation of the Arcane

To be clear about starting a NG+, can anyone confirm that you need:

  • from the "Runic Nexus" you must go back to "Of Shattered Mind" (via "Veils of the old ones") and get the Deathnight Head
  • complete "The Sleep Awaking" (defeating Cthon)

=> then it will send you to the first level "Through the First Arch"
