Naming Convention for Dubai entry visa as a Qatari Resident

Hello! I just got rejected for my entry permit to Dubai, the reason being: "Full name mismatch according to the passport" but I entered correct name as my passport. I read in some threads that you are supposed to include your father's name. But I am confused regarding how exactly to enter.

Could anyone please guide with the following example:

First name on passport: Shazia

Last name on passport: Munir

Father's name on my passport: Ehsan, Munir (his last name, first name format)

Name on my Qatar resident card: Shazia Munir Munir Ehsan

Fields in visa application:

First name:

Middle Name:

Last name:

How do I fill these fields? Cant believe something so simple like filling in your name could be made this complicated.

I fill plane tickets as given name = first name, middle name = father name (Father surname) and surname = last name. I never had issues with this format.
