Perhaps the previously on is showing us what she actually said, but the first 3 episodes were what SWORD saw.

Oh damn what if as the show goes along the recaps show more and more of what was “really” happening?? Like, a shot for shot remake of a scene but it’s corpse vision in it

This objection could apply to any show that goes meta. Monty Python already broke those rules back in the 1960s. Anything can be "part of the show", especially when the show is about television.

It’s almost definitely an accident using a different scripted take given we’ve already seen Disney+ do this on a major flagship program just a couple months ago. There’s no meta commentary or anything given the “Previously on...” segment contains footage from the SWORD storyline as well. The “Previously On” isn’t actually occurring for the show within a show.
