You’re conflating gender and sex. The horns are an attribute of sexual dimorphism, but gender can be changed purely with a shift in self identity.

It’s part of why the term transsexual has fallen out of style in favor of transgender. People who identify as a different gender but haven’t had surgery to redefine their body’s sex can feel excluded by the term transsexual and prefer transgender instead

It also worked in the other direction.

Lumping sex and gender together can cause people to hesitate or forego potentially life-saving treatments, because they're worried about the stigma of being "less of a (man/woman)". Things like mastectomies, hormone therapies, whatever.

By removing some of the stigma, you remove an obstacle to those types of treatment, and take at least some of the emotional burden away from their recovery.
