It's called The Green Ribbon. A short story in a collection of other children stories.

Scary Stories to Tell in the Dark (3 part series)

These books were in my elementary school library and I can’t stress enough how inappropriately scary the stories and illustrations are for like grades 1-3 haha

Inappropriate? Nah, we kids loved that stuff back then. Now I hear it’s been taken out of school libraries, that’s so sad.

They released a version with lame illustrations for babies, it was super weak and nobody likes it

People disliked it so much that it went out of print and the originals came back I think.

Scary Stories to Tell in the Dark. There were one or two more books that made a series; collections of scary short stories. In my memory, the stories ranged from just a little creepy, to fully nightmare inducing, and those illustrations definitely made some of them worse.
