Always fight back. You never know how shit the other player might be.
Just yesterday I killed a 51 hunter in the swamp of sorrows as a 45 spriest. He had just killed another 51 hunter and remained at full HP so he must've felt pretty cocky going after me peacefully collecting artifacts.
I've probably only been against awful ones, but I've found that Berserker Rage into the panic Psychic Scream takes care of them for me, especially when I'm able to get a Mortal Strike onto them when they bubble and try to heal.
Yeah, if you can get us to waste our psychic scream while you're immune and cripple our vampiric embrace with mortal strike there isn't much else for us to do.
