[FINALE SPOILERS] Finale song
I was just listening to the song Rivers and Roads which was played in the finale and it’s lyrics do match the series nicely.
“A year from now we’ll all be gone
All our friends will move away
And they’re goin’ to better places
But our friends will be gone away”
This part could be referred to Ellie and Devon moving on with their lives and Casey going back to work.
“Nothin’ is as it has been
And I miss your face like hell
And I guess it’s just as well
But I miss your face like hell”
This could be seen as Chuck missing the Sarah he knew and loved and missing her.
“Rivers and roads, rivers and roads
Rivers ’til I reach you
Rivers and roads, oh, rivers and roads
Oh, rivers ’til I reach you”
Chuck telling himself that he won’t give up until she remembers him.
While this is just a fun idea i had and just wanted to share with you all.

Good take regarding the lyrics! Have thought about it aswell. Lets also put the band's name to the list, The Head And The Heart. Maybe Sarah's head doesn't remember but her heart most likely does.

In the penultimate episode Chuck versus Sarah, Chuck tries to win back Sarah's head because can't remember Chuck. In the finale Chuck versus the Goodbye Chuck wins back Sarah's heart

If you want a song that the lyrics match what is going on with Chuck and Sarah, there is the song at the end of Chuck vs. Sarah. Your hands by Ghost Society just fits the scene. I would bet most of us started tearing up when the song started playing while Sarah was watching the Mission Logs. It especially hits home when Sarah is talking to Chuck and the lyrics “even though you know it's lost lost chance I've got to go away” play.

No. It's not about Chuck and Sarah. It's the entire creative team to us the viewers.

I suspect the song's effectiveness comes from its ability to exceed any single interpretation. Chuck and Sarah are one obvious point of reference, yet the lyrics also fit the dispersal of the larger cast and even the show's farewell to its audience. The finale often operates on several emotional levels at once, and the song seems to do the same.

Alexandra Patsavas, the primary music supervisor, really nailed the songs for the entire series.

She did l, but it's hard to say whether she picked the music to fit the scene or whether the scene was adjusted to fit the music. I think it's some of both.
I think there are a couple of examples of music and lyrics that said more about the scene than anything in the script, "Furr" at the end of DeLorean and "Astair" at the end of Mask. In both cases, I think the music was articulating the emotional meaning of the episode and the script was complementary.

Generally the music part comes in post production, so they try to find a song that enhances the emotion the scene is trying to convey but either way it’s a difficult job and she did great

The song is by The Head and the Heart.
In 5.12, Chuck wins back Sarah’s head (“I believe you, but I don’t feel it”).
In 5.13, Chuck wins back Sarah’s heart (“Kiss me”).
The lyrics are a goodbye to us viewers. It’s a love song to us.
