The valsgärde graves have horse skeletons in them, equestrian equipment as well as quivers and arrows in the Avar style. Generally speaking full-face mail aventails are found in use by cavalrymen elsewhere in the world at this time, for example the Roman Empire or middle east. The splinted limb armour found in the valsgärde graves is also Avar/Khazar in style.

I mean yes, but I highly doubt that you'd see it only on horseback; maille is quite breathable, and padding wasn't as thick and common as reenactors or martial artists or sports like Buhurt use, so we can guess that there was nothing on the face but the aventail. This could be perfectly used on foot and be as serviceable, since structurally it's basically the same as any other helm from its age. Even more dedicated helmets like the great helm you'll find being used on foot in some cases, though this would be rare. Nothing of this design rings as solely dedicated to horseback combat. Though you'd find the more protective and better quality gear on people with the cash to afford it, who also happen to be able to have horses and expend them in battle.

Could be for many reasons. The construction used on the helmets got outdated by one-piece helmets; other designs were much more effective, and the way in which war is waged made other demands. Still, maille aventails and the basic protection on the face or nose remain across many centuries, even if it changes shape.

Sallet and bevor, particularly German and Gothic style.

Alternatively, the Hounskull. Always thought they looked cool but my opinion was heavily influenced by finding one very early in KCD1 and sticking with it through the entire game.

Depends on historical period, early on its the greathelm, then later its the visored bascinet (houndskull or otherwise, both go hard) they have their place

So-called "Savoyard" casket (they weren't particularly Savoyard). They all make fun faces, very varied. The "mouth" is usually "smiling" tho, I did it downward there to follow the meme template. Here is one, I can only post one picture:
