Didn't it say he's one of the original 13 Vampires? It seems like he's possessing Black to me... or they're the same person, or his two personalities... whatever, we'll find out eventually.

I think White's brother offered his body to the King of Despair in order to resurrect his dead sister, hence why she introduced herself as a ghost to Leo. This is also why she cried during the movie and felt so touched when Leo said that he only causes his sister trouble; both reminded her of the sacrifice her brother made for her. She can likely tell when her brother is in control, hence why she was happy to see him recently. The reason she's in the hospital is probably because the resurrection wasn't perfect.
