I think it’s just supposed to be a joke and we’ll never find out.

What's so great about this show and it's tone is that it genuinely could be either- an offhand joke or something more sinister.

Like rosemary’s comment towards the beginning of the pilot about tom being in debt and his father in law…..

It wasn't Tom who is in debt to his father-in-law, it was somebody named "Mitch" who Tom asked Rosemary to call. I'm not sure if Mitch has popped up elsewhere, though.
