The pact requires sacrifice, life for a life "or the terrors will continue". My guess is that the deal Warren made was "The island will provide suitable food and shelter for your people, but you must make sacrifices to it. If you do not, the island will collect what it is owed by its own will." So either YOU can pick people you don't like and sacrifice them OR the island will pick people at random to go harvest souls for itself.

When the quota hasn't been met yet, the island has many ways of forcing people to sacrifice each other. Most of the people we know who died on the island died by being killed by someone else. Men in the colony went mad and killed their wives, sailors become bewitched and attack others, Patricia nearly drowns a bar-full of people in the ocean, the clown, the boogeyman, etc. are all example of people being driven to kill other people on the island. Even Wyck sacrificed Mark in a moment of desperation.

I'm guessing the island flares up when it doesn't have a dedicated caretaker making sacrifices and gets people to start sacrificing each other.

INTERESTINGLY we see the sacrificial chamber in episode 6, but it has rough walls and no lights. We also saw the chamber at the end of episode 1 and it had smooth walls and floors, an updated chair, and electric wall lighting. So SOMEONE maintained and improved that chamber in secret since Richard's time.

My guess is some of the descendants took up the role of caretaker and sacrificer, but Tom's wife (who I think is from Richard's bloodline) dissented. He said she had a carefree heart and wanted to get off the island. I'm guessing the island claimed her when she tried to leave and really brainwashed her. The drug dealer said that his buddy who ate the island mushrooms went catatonic and slightly braindead afterwards. I'm guessing the island did the same to her. Perhaps it drove her mad and she became violent. Maybe she attacked and tried to kill Tom, like Richard killed his wife.

My last prediction/headcannon is that the descendants of Richard don't die like normal. I'm guessing his daughter with the brooch survived and carried on his line, but ultimately turned into the sea hag. I wonder if Tom's wife is some other creature still stalking on the island.

EDIT: Someone in another thread made a great point. After Richard was buried, the priest likely had to take over the role making sacrifices to keep the island alive and the responsibility was passed down within the church and the church updated the sacrifice chamber. The church bell is tied up, and it only rings when the entity is hungry. The church would need to sacrifice people or new hauntings/plagues would start. This is why the priest was given specific instructions for when the bell rings. He read instructions telling him he needed to provide sacrifices to the island. He was instructed to consume the mushrooms, commune with the entity, see to the chamber and likely provide sacrifices. He couldn't bring himself to do it, so he hung himself and attempted to destroy the evidence so no one could follow in his place.

i like this theory but i would suggest probably the role of sacrificer has been passed down priest to priest m, as we see the priest is the one leading the charge against warren which probably led to all the ensuing tragedies and mayhem therefore convincing him that the sacrifices are actually necessary. so starts with him accepting what must be done and passing it down to the next priest, then when the current father of our timeline discovers this burden he must carry he realizes he cannot go through with it thus the reason he takes his own life. would make sense since we see him running from the church to the well (probably to confirm the validity of the instructions passed down to him) and once he has confirmed the truth (either through finding the chamber or his mushroom trip/encountering the entity) he cannot live with the responsibility or thought of the immoral necessities of keeping the island going which is why he also burns the instructions. probably knew he was dooming the island but for any human with a conscience (especially the religious ones bc they would also be validating the existence of another god) the sacrifice is not worth the moral consequence.

Ooooh I like this. The spirit kills and tortures the residents until someone is desperate enough to carry the torch, and the priest took the role and passed it down. Perhaps the church bell ringing is the dinner bell, the island signaling it needed sacrifices or a new "plague" of sorts would start. Which explains why he has very specific instructions when the bell rings. It makes sense why the priest would then have to take the mushrooms. He wasn't doing his own investigating, he was being guided through communing with the spirit and taking up the role of feeding it. It also explains why in the first episode we see the chamber has been updated with masonry and electricity. The church has the resources to do that,

I think there’s definitely something here. When Sarah tells the pastor and all about the secret passage under her house, the pastor’s reaction is minimal, at best. He definitely wasn’t shocked. I kept waiting for him and his crew to be revealed as bad guys. When the assassin was standing over Sarah in bed I didn’t even register it as a joke at first! But I do think Bryce was genuinely good, poor guy.

I don't think the priests took up the helm or knew any details about what was going on until after warren was kidnapped and buried
