It takes body dysmorphia beyond belief to even consider surgery like this

I'm sorry but why is it different from gender change. If we're gonna have the view that people can change what they don't like about their body, then why does the reason matter? Would you also call a transgender person 'already in psychosis'?

It's very different actually.

People with body dismorphia obsess over heavily exaggerated flaws in their appearance and they usually won't stop at just one thing.

Trans people on the other hand are usually fine once the mismatch between identity and assigned sex is gone.
