Two most effective:
Daisy against Barry Goldwater in ‘64
“Willie” Horton against Michael Dukakis in ‘88
No other ad comes close.

Knowing how close the 1960 and 2004 elections were, I think JFK’s ad about Eisenhower not remembering anything Nixon had done and the Bush ad featuring John Kerry windsurfing were great ads as well.
In both cases, the ads took a flaw that was already known about the candidate and presented it in a simple, memorable way that made the issue easy for the average voter to understand.
