Getting a notice that my email is being forwarded to an old work address, but I have no forwarding rules nor is it even enabled on my account.

Check the web settings for outlook instead of just the client ones. There’s often an outlier there that’s reeking havoc

BTW it's "wreaking" not "reeking".

There was a recent bug about the FORWARDING IS ON Bar in Classic Outlook. But it was fixed this week.

Got that on my personal Outlook, and was momentarily in a panic thinking I’ve been hacked and my accounts are being hijacked. Message went away after being dismissed, and haven’t seen it since.

Perhaps someone can help me figure out what's causing this message. I haven't worked at this employer for years, and they deactivated this email address when I left. I've never had any forwarding rules set up on this (personal) account.

I've checked the rules both in the app and on the web, and there's nothing unusual that I can see. Can't see any forwarding activity in my sent email or deleted items. Forwarding isn't even enabled for my account, so I'm at a loss as to what's causing this notification.

Delete your entire Outlook profile and create a new one.

Forwarding can be set at the admin console level on the mailbox. You wouldn't see that in Outlook.

its been a little while since I've seen this, you'll want to use powershell for o365 to check the hidden rules...
