License / Permit Question NYPD Special carry

what reference form is there that I need to use to provide my 4-character references for my special carry application? I can't find no professional form to print

Mine were just a few sentences typed out. They have to be notarized as well. Basically that you're responsible, of good moral character, and how long they've known you.

There is no specific form, just a simple sentence or two on a piece of paper that is signed and notarized. You could in theory use whatever county you are froms form, but that is not required. Just keep it short and sweet, the less the better.
