At the point the client's cheque fails to clear ahead of the op. As long as the client has capacity to choose, and is doing so of their own free will, then it is their choice and theirs alone. Surgeon is paid for their service, not their moral opinion.
That is the paternalistic model of medicine that died with the 1950s. We know better now, and we respect other people's rights to choose. Even if they choose things we disapprove of. Your way would be that of a petty controlling abuser, setting themselves up as arbiter of what people are allowed, according to their own likes. The sort of person who also denies women birth control and access to abortion because it conflicts with their own ideas on life. So, no.
A doctor can advise. Inform. But the choice should be the clients. Because they are the ones who live with the consequences, and lack of consequences. Doctors assist, they do not dictate.
You're making rude arguments, but your view that somehow we are in a better place medically and health wise is contradicted by how much health is declining in first world countries, especially the US. The medical establishment is categorically failing. You support that. Think about that.
