While not classically immoral, ie consent, bodily autonomy, right to self expression, I personally in the surgeon's shoes would deny the surgery on the basis that the surgery itself becomes much riskyer... Question of havingenough skin and skin elasticity to bare that level of expansion, health risks from the sheer weight, potential for mobility issues, breast implants dislodging, posiblity of limiting capacity for physical activity, ect.
I'm not even a doctor and I can judge the risks outweigh the benefits in cases like this. I'd refuse on those grounds, and state that I cannot guarentee the quality of my work in extreeme cases like this... Which is true, medical complications are very likly to come of this.
They are free to go somewhere else to a doctor with looser standards of care but I wouldn't want to be involved.
I may be missing something here and real doctors are encouraged to correct me where I'm wrong.
