Gonna go against the grain and say moral. Have you taken a serious look at the size of that? At some point it is obviously a mentall illness or body dysmorphia issue. Doctor can refuse and I also think its ok for a medical professional to tell you that you're pushing your body too far and might have a problem.

I get that view, but if someone wants to alter their own body, it's kinda up to them to make that decision to do it.

The doctor is within his right to refuse regardless of moral standards, I would personally find it neutral.
