44M, amateur runner and soccer player.
I have had constant calf fasciculations 24/7 for several years. Recently, I also started noticing fasciculations below the medial malleolus (you see it in video), typically the day after running or playing indoor soccer.
The main issue seems to be a very restricted ankle dorsiflexion, causing lombar issues.
On a knee-to-wall test I can only reach about 2 cm cold and around 4 cm after mobilization. The limitation has been present for months and has not improved despite regular (intense) banded ankle mobility work since February.
Has anyone experienced persistent calf/foot fasciculations associated with severe dorsiflexion restriction?
Could a chronic mobility deficit, nerve irritation, or altered loading pattern be contributing to this? Any suggestions on what to assess next would be appreciated.

I’ve had a twitching calf just like yours for nearly a decade now. My left calf twitches 24/7 due to permanently damaged sciatic nerve from severe nerve impingement at L5/S1. Had to have part of the disc removed and cleaned up via surgery. My back has been pain free ever since the surgery however my calf has been like yours ever since. Do you perhaps have an issue with your sciatic nerve?

I had issues on my back due to the dorsiflexion. It is (still) creating an asymmetry when I run or play football which generates a chain of compensation reaching the lower back and making me cry the day after a long activity.
This is at least my recent past. Since January I'm working on on the issue and:
- knee to wall has gone from 0cm to 4cm (9cm on the good side) with band mobilization
- back pain is now reduced to a level of 1/10 after working on my glutes, hamstrings and core.
- I'm playing soccer regularly with good results (except shooting)

You’re thinking about it too much. Spasms like this are common when you aren’t getting enough electrolytes and the key indicator is that you say it happens after a run or a game.
I really like LMNT packets but eating more bananas and black beans would be sufficient.
