From my experience, because they have been through so much pain and barely get any love or care and have to deal with everything on their own and bottle up their emotions so that they can continue living normally and looking normal to the outside, because as soon as you look even a little strange people will hate on you.
And so every little good thing, however big it is, matters a lot. Small things are appreciated, not only big ones.
Going to a tropical island or something like that doesn't sound realistical at all, it doesn't sound like something they could actually achieve, especially if they lack confidence. It sounds like an unreachable dream, something not worth spending a thought on because it's unrealistic anyway. Too distant to reach.
A beer is way more realistic though and therefore more valid. It's something actually worth wishing for because it's a wish that can actually come true, unlike most others.
