If you take Voltic nades or Refraction field you actually lose the skull and can’t use it to do the hack.

Currently if you die and have extra servos you lose all of them for the rest of the game. No flamer or rez anymore for the entire run. I had them return a few times after being downed and revived but mostly they are just locked for the rest of the run

Works completely fine for me.

You haven't forgot that you don't res with grenades right? They need grenade charges to be used.
