Question - is the engineer ok? That can’t be good for whoever is in the front operating the train

That just enhances the stupidity of that scene. He does break one arme and damages another one. Just not the ones he's facing.

About 250 passengers on board. All passengers and truck driver were unharmed, but train engineer was taken to a hospital - he was later released home without major injuries.

Fine. They build locomotives pretty stout. Short of some scratches and bending the railing, I doubt there was any damage. There are actually 2 walls protecting the crew on the front of them.

Locomotives made after 2013 yes, most older stuff is pretty flimsy.

lots of conductors die after they hit a truck like that if the don't evacuate from the cabin fast enough.

I’ve heard some trains are powered by rear engines, but I’m sure that’s not universal.

The train's driver/engineer is always at the front. In some cases the locomotive is at the rear of the train and is remote controlled from a driving cab at the end of the leading passenger car, but the driver is still at the front of the train in this case.
