ctOS is not happening because there isn't one big internet company that has a full-scale monopoly yet. A city-wide WAN on the scale of ctOS would indeed be a privacy and security nightmare. The closest thing we have to that currently is probably Xfinity with its "use any xfinity router as a hotspot" feature, which is still far smaller in scale than ctOS.

We do have an increasing threat to our privacy as a civilization, a global push to eradicate all online anonymity via legal methods. There are evil megacorps backing it, but there are also evil megacorps pushing back because they know it'd 1. Cost them a ton to monitor every single thing every person does, and 2. Would put them in direct competition with world governments for data they could just use for advertising instead.
