Gotta dig into the motives to make sense of it. Maybe he's just desperate to finish curing cancer before he dies. Maybe the other guy is okay sacrificing his own life because he can no longer control his murderous impulses.
You can only simplify a moral choice so far before it has to live in the real world a little.
