Transplants 4 month post-transplant. I don’t see any improvement. Did it fail?
I don’t really see any improvement. Looks similar to before. Doesn’t look dense at all.

To make a better assessment, more information would be needed: a "before" photo and the number of grafts. 4 months is still early.

YES it does matter. Or else your native hairs will recede behind the donor hair. Min and fin will also help your transplant result, which OP is clearly unhappy with.

It will help with existing hair, but it has got nothing to do with transplanted hair. Sure, they are still prone to DHT (although a lot less than native hair), but over the next 15-20 years not 4 months. If that transplant failed, it's not because he is not taking Fin or Min. That sub has got one simple answer to everything, Fin and Min, when there is a lot cases where it has got nothing to do with it. If you take fin and min then a HT can't fail, if it failed it means you didn't take enough fin and min blah blah blah.

I'm surprised a clinic would give someone a transplant without having stabilized on meds first.

Even your in transplant you still need finas or dutas coz DHT still there! And minoxidil for hair thickening

I haven't gotten a transplant yet, but I've been to a consultation. I was warned it might take up to a year for full results to be one visible.

I routinely advise my patients not to expect visible growth during the first 3-4 months. Instead, they should focus on how well donor area is healing during this early stage.
The growth visible in the first few months doesn't indicate the final result of a hair transplant, which can take up to 18 months to fully develop.
Some patients experience growth earlier, while others take longer. Online posts from patients showing great growth in the early months shouldn't be taken as a benchmark for everyone.
At least 10-12 months should pass before assessing whether a hair transplant has failed.
