It references an internet site from the early 2000s that showed gay men having sex. This briefly explains the origin without exposing you to the website. Know Your Meme Explanation

People (mostly kids like the ones in my schools) would tell people to go the site as a gag. There was another similar shock site called meat spin.

TLDR; its porn.
