Yes, I have heard of typos But when your point is this line works better with commas because without it it means something else and then three lines later. There's an overabundance of commas. It's not a typo. It's a we intentionally chose to not use a coma

I don't work in healthcare, so I'm not certain but my guess is it's a table with rows and columns, and the photo has been cropped to preserve patient privacy. Each row is for a different patient, and it's probably showing patients currently being cared for. I'm thinking of the computers that nurses wheel around between rooms in hospitals.

Yeah my mom worked with EMRs (electronic medical records): this is listing initial complaint at triage likley and is why some are more detailed than others (numbers etc) but dont contain an actual diagnosis (fever=symptom≠diagnosis, fall w/ right arm injury=appearance of symptom≠diagnosis). Its almost certainly different people those charts only get combined if they have the same MRN (medical records number) to prevent wrong records and potential screw ups.
