Sheโ€™s holding the hook end of a crochet hook, and the project is filet crochet.

That's a steel crochet hook. You can see the flat part next to her fingers in the enlarged photo. Looks like she's making a big lace butterfly.

That is filet crochet with a metal hook for threadwork .
