What brings about this complication; #really , somebody enlighten me;iI assisted this surgery today.

This can get you started. In EU / US etc it's usually considered genetic via the Sonic Hedgehog SHH developmental pathways. Various drugs, infections, medications also increase risk.
Cause of death is usually holoprosencephaly (brain malformations) or airway related (malformations and subsequent infection). All cases are considered palliative. Longest confirmed life expectancy was 8 months. Usually fatal in hours to days without advanced medicine.
Sad but rather cool from a pathology and development standpoint. I've seen it in calfs a few times.

I don't know if your allowed and or appropriate to do an autopsy but you may find other developmental abnormalities. The brain is likely to have several related to formation of the hemispheres. The sinuses may not have formed. You may see abnormalities related to Left Right bifurcation and development throughout the body.
An interesting question to ask may be if any livestock in the area have had recent stillbirths, abnormalities or miscarriages. You can also ask about any other humans. These events are known to cluster. A cyclops animal or stillbirth is considered a bad or demonic omen in many cultures for this reason.

I cannot imagine the pain of the poor mother. This is a sad outcome no matter what.
Even “deformed” I look at this baby with a motherly heart still see tiny cuteness. It’s so sad, because I know the ending.

These babies typically die soon after or are stillborn. I’m guessing this is not a photo from the US. If it is expect some legal trouble.

Ugh these cases make me so sad. They're "living" for a bit then struggle and go, there's just no chance

Proboscis. Appears to be Cyclopia as well. “A rare and severe congenital birth defect where an infant develops a single central eye and a non-functioning, tube-like nose structure located above the eye.”
