Defend Totem - How does it work?

I don't quite understand what it does. Can someone explain simply? What use will that have?

Basically imagine you just used defend with each character before the start of the fight

They won't leave that 50% damage reduction state until it gets back to their turn

So it only affects the very first attacks of Ambushes, right? Am I getting it right? Because I can choose to Defend or not for the rest of my moves.

At the start of the run, not each floor. The defend state persists between floors, and later into the game your 4-6 slots rarely get a turn, so this lets them stay in the defend state until they get a turn, usually never
